Offers community habilitation, respite, and intensive behavior services for individuals with Autism Spectrum disorders or a related developmental disability.

Offers short-term respite services, educational workshops, training, support groups, and various therapeutic services to adults taking care of a senior age 60 years or older, or a grandparent over 60 caring for a child under the age of 19.

Expanded In-home Services for the Elderly Program (EISEP). Non-medical service that assists people who are not able to meet their needs independently. It is designed to serve a population of seniors who are not eligible for Medicaid, but do not have sufficient resources to pay for private care. Services available can include help with eating, bathing, reminders to take medication, housework, laundry, shopping, errands and paying bills.

Helps with housekeeping or personal care so older adults can stay in their own homes for a longer length of time.

* Provides home health aides, licensed nurse practitioners, registered nurses, attendants, and personal care aides to work and oversee medical treatment in the home. Some of the services provided are: Medication reminders Grooming and oral hygiene Bathing and dressing Mobility issues Meal preparation Laundry Housekeeping Errands Companionship* Provides a full range of services under the TBI/NHTD Waiver that includes the following: Home and Community Support Staff, Community Transitional Services, and Moving Assistance and Transportation. * Offers non-medical transportation services.* Provides care management through Health Homes of Upstate NY (HHUNY) and Children's Health Homes of Upstate NY (CHHUNY).* Offers tuition free certification for Personal Care and Home Health Aides.

Foster parent must be at least 21 yrs, pass background check, receive ongoing training and support. Foster parent receives daily stipend; clothing allowance; and medical, dental, and orthodontic coverage for children in their home. Can be matched with children ranging from birth through age 21 yrs, including pregnant teens.

Provides respite and support for families who need a break as our staff supervise, socialize, and assist with family members with recreational activities planned within the home and in the community.

Offers respite care for individuals with disabilities to allow their families and care takers time to address personal matters:

Full continuum of care- from independent living in an apartment, to assisted living offering both memory care and assisted living options to accommodate varying needs, skilled nursing care. Also offers a wellness and fitness facility with certified trainers ready to help residents of all abilities,and a short-term rehabilitation unit.

Provides relief services to primary caregivers of individuals with developmental disabilities and the service is provided for the individual with the diagnosis and respite service is provided at home. Offers assistance with community integreation and independence.

Program offers assistance, information, education and referral about benefits, housing, nursing home advocacy and diversion to avoid moving into a nursing home, respite services for care givers, consumer-directed personal services, traumatic brain injury services, and a personal care bank for toiletries and supplies for families in need.

* Provides short-term out-of-home crisis stabilization for youth who are experiencing an emotional crisis.* Brief stay (up to seven days), in a host home, for crisis stabilization and then return home.

Provides support to families to help relieve them of their caregiving duties.

* Provides programs and services for people with intellectual and/or other developmentally disabilities and their families.
