Some of the services provided are: Medication reminders Grooming and oral hygiene Bathing and dressing Mobility issues Meal preparation Laundry Housekeeping Errands Companionship* Provides a full range of services under the TBI/NHTD Waiver that includes the following: Home and Community Support Staff, Community Transitional Services, and Moving Assistance and Transportation. * Offers non-medical transportation services.* Provides care management through Health Homes of Upstate NY (HHUNY) and Children's Health Homes of Upstate NY (CHHUNY).* Offers tuition free certification for Personal Care and Home Health Aides.

• Provides caregiver services for developmentally disabled individuals living with family to provide a break. Respite may be provided at a site or in a family's own home.

Provides care and supervision to individuals with a developmental disability and their siblings in Onondaga County. Parents and guardians can use this ongoing respite service for personal, social, recreational, or health care needs. Providers are individually matched with families based on provider experience and family needs. Open to families who do not already have HCBS Waiver or Self-Directed Respite services.

• Offers a 24 hour Helpline that provides reliable information and support to people with memory loss, as well as, caregivers, health care professionals, and the public.* Offers free community education courses on Alzheimer's disease. * Provides social activities to encourage individuals with Alzheimer's disease to continue engaging in the activities they enjoy. * Offers care consultation that provides needs assessment, planning and problem solving, plan development and support.* Provides education regarding prevention of wandering.
