The Long-Term Home Health Program provides home health care services that promote the patient's quality of life by providing those services in the most appropriate home care program, including adult in-home respite care, and home delivered meals.
On-going programs include: Children's Housing (both hospital and retreat) and Hands on Adventures.

* Offers the following services:
  • Adult Day Services - Provides nutritious meals, social programing, supervision, personal care services, community outings, assistance with activities of daily living and door to door transportation (Oneida and parts of Herkimer counties only).
  • In-Home Companion Care - Offers companionship and assistance with everyday activities.
  • Caregiver Services - Provides community education, information, and referral for caregivers of the disabled or elderly.
  • Ombudsman Services - Provides information and advice to Nursing Facility residents who have issues with their services.
  • Open Doors Transition Program - Informs Nursing Home residents of community based living options and explains transition process.

Encompasses a wide range of programs including Service Access Assistance, and assisting families pursue OPWDD supports and services. Staff provides information and referrals for services, informal counseling, advocacy, ongoing support, and assistance to find funding sources when no personal funds are available.

Provides care and supervision to individuals with a developmental disability and their siblings in Onondaga County. Parents and guardians can use this ongoing respite service for personal, social, recreational, or health care needs. Providers are individually matched with families based on provider experience and family needs. Open to families who do not already have HCBS Waiver or Self-Directed Respite services.


* Offers someone who can assist with a listening ear or a helping hand when the primary caregiver cannot be there.* Provides respite care that offers short term breaks that relieve stress, restore energy, and promote balance in your life for the family.* Provides shopping assistance for groceries, personal items, gifts and prescriptions.* Provides meal preparation, laundry service, making beds and changing linens, tidying up, running errands, and medication reminders.* Offers transportation to doctors' visits, dialysis procedure, trips to the hairdresser or barber, or just visiting a friend.

The Rockland County Office for the Aging offers a free week-long stay at an assisted living facility. A new home care component to the program allows an individual to remain in their home with an aide for a specific period of time. Respite is available for individuals 60 and older who reside in Rockland County and have a caregiver. This program is subject to availability and funding.
Offers a skilled nursing care unit for veterans requiring extended or long-term care due to age, illness or disabilities. Also provides hospice and palliative care, including out-of-home respite care.
