Hearts and Hands is a volunteer based non-profit organization that provides free escorted transportation, companionship, and in-home supports for older adults in communities throughout Erie and Eastern Niagara Counties. Our care recipients are often individuals who stopped driving due to age-related medical conditions or for financial reasons. Recipients may also need assistance at home with light yard work and home maintenance, or companionship to renew and refresh their caregiver. Our elderly neighbors are often lonely, isolated, and in need of friendly human interaction. In response to a recent community assessment and a national assessment from AARP, Hearts and Hands has designed our program to address social isolation through all three avenues of service: transportation, household supports, and companionship. Hearts and Hands volunteers may be the only people our care receivers see during a week, and their company is, in and of itself, a bright spot in daily life. Hearts and Hands is a best-efforts program. Services are provided by volunteers who give their time as they have it available. Offers information, referral, and assistance to obtain needed services. Provides counseling and consultation on caregiving issues and long term care planning. Offers support programs such as In Home Respite Care, Personal Emergency Response System, and Adult Day Services.

* Provides respite care for families who have children with emotional, behavioral, and/or mental disorders. Planned group respite events takes place in the community. These events are generally offered to identified children and siblings, on a regular schedule, predetermined by the parents and staff. In-Home and Community respite care is available on a planned hourly basis to all of the children in the home or at a community activity.

* Assists individuals with developmental disabilities: In-Home Respite - gives caregivers and those served an opportunity to spend some time apart Day Habilitation - includes training and assistance in personal care, basic safety skills, social and recreational activities, relationship building Community Habilitation - community living setting with skill-building including money management, menu planning, and food preparation, chore completion, and recreation Individualized Residential Alternatives (IRAs) - provides a group living environment with services tailored to each resident's individual needs

Starting with the individual's strengths and interests and emphasizing personal choice, The Arc Otsego offers comprehensive network of community-based programs and services that encourage individuals with intellectual and other developmental disabilities. Assisst with medicaid service coordination, obtaining respite services, skill training assistance, environmental modifications, adaptive technologies and guardianship.

Foster parent must be at least 21 yrs, pass background check, receive ongoing training and support. Foster parent receives daily stipend; clothing allowance; and medical, dental, and orthodontic coverage for children in their home. Can be matched with children ranging from birth through age 21 yrs, including pregnant teens.

Program offers assistance, information, education and referral about benefits, housing, nursing home advocacy and diversion to avoid moving into a nursing home, respite services for care givers, consumer-directed personal services, traumatic brain injury services, and a personal care bank for toiletries and supplies for families in need.

* Offers a 24 hour Helpline that provides reliable information and support to people with memory loss, as well as, caregivers, health care professionals, and the public.* Offers free community education courses on Alzheimer's disease. * Provides social activities to encourage individuals with Alzheimer's disease to continue engaging in the activities they enjoy. * Offers care consultation that provides needs assessment, planning and problem solving, plan development and support.* Provides education regarding prevention of wandering.

*Adult Programs:

• Education Support Services - assistance to individuals who want to start to return to school
• Pre-vocational Services - Time-limited services that prepare individuals for paid or unpaid employment
• Intensive Supported Employment - Assists recovering individuals to obtain and maintain competitive employment
• Ongoing Supported Employment - Supported provided to individuals after they have successfully obtained employment
• Family Support and Training - Enhances the skills of family members and supporters so they can support the recovery of a family member with behavioral health issues.

*Children and Youth Services:

• Community Habilitation - assistance with learning social and daily living skills, and health related tasks and participate in community activities
• Day Habilitation - assistance with learning social and daily living skills such as building relationships, cooking and communication skills
• Caregiver/Family Support - Training and education for families and caregivers to make informed and empowered choices
• Community Self Advocacy Training and Support - Assistance to understand developmental, medical, mental health, and/or substance use needs; assistance with difficulties while participating in community activities; self-advocacy training
• Prevocational Services - learn skills to help get ready for paid work or volunteer work
• Supported Employment - ongoing support while getting a job and while working
• Planned Respite Services - provide short term relief for families/caregivers and support the child

Provides activities for children and adolescents ages 17 years and younger with an emotional, social and/or behavioral disorder. Offers respite to children of adults with a psychiatric disability.

Recruits and trains individuals to care for children and adults with disabilities to provide respite care from constant care of dependents with disabilities. Assists in getting respite care.

Assists elderly to remain in their homes and to participate in family and community life as long as possible.
