Provides daycare and preschool services for children with disabilities aged 0-5 years old. Provides respite and school age services for individuals with developmental disabilities aged 5-21 years old.

Offers respite services for caregivers and families caring for someone at home, and need time off or away for short periods of time.

Provides Family Care and Individual Residential Alternatives (IRA) for developmentally disabled persons ages 18 and older who are HCBW eligible. Intermediate Care Facility is available for those who are Medicaid eligible. Transportation is available for all. Supportive apartments are also available, with limited transportation. Provides respite care allowing families of developmentally disabled persons to take a break from the daily routine of care-giving duties. Currently, two locations in a certified home can offer respite for as little as one day or for a few weeks. Reservations are encouraged; however, emergency accommodations are available. Provides At-Home Residential Habilitation service that assists individuals in improving the skills necessary to live in the community while living at home. Qualified staff work one-on-one with individuals to help them learn socialization, recreation, daily living, and mobility skills.

Provides an opportunity for temporary relief for caregivers/families of individuals with developmental disabilities. Provides after school care and care on Saturdays outside of the family home.

Offers respite care services for caregivers or families who need a break from routine of caregiving, or need time off.

Provides family support services to help families care for their children who have a serious emotional disturbance (SED) diagnosis or who are at risk of being diagnosed. Services include: individual meetings, support groups, workshops and family engagement activities.

Offers respite or short-term stays for families or caregivers of Alzheimer's patients who need a break or take time for a vacation or do other things.

Offers diagnostic and emergency placement for sibling groups removed from their homes or in the process of placement transition. The program can accommodate 12 children and offers a 24 hour professionally supervised environment. The program provides crisis and preventive respite services for youth and their siblings. Referrals must come from Nassau or Suffolk County Departments of Social Services as well as the Mental Health Crisis Respite Bed System.

Provides Family Support Services as well as Waiver Services to eligible participants. Provides Family Reimbursement, Behavior Management (not intensive), and Service Coordination and Referral. Also offers Recreation/Respite and Community Habilitation Services.

Provides in-home visits, counseling, and resources for caregivers and individuals with Alzheimer's and Dementia, including specific assistance and training, monthly support groups throughout the county, articles and information specific to caregivers, and respite care (in home or facility based). Provides supportive counseling for family caregivers through Family Services of Jamestown. Support group meets at Christ First United Methodist Church, 663 Lakeview Avenue, Jamestown. Provides hourly respite and overnight respite (out of home) in planned or emergency situations (transportation is arranged whenever possible). Agency also provides residential habilitation, assistance with transportation issues, linkage, referral and assessment, intake, and advocacy for developmentally disabled children and adults living with family.

Provides recreational activities for children and young adults with a disability who require minimal supervision. Referral required.

Provides a unique night program for those memory-impaired seniors who suffer from sleep disturbances associated with dementia.

Offers a skilled nursing care unit for veterans requiring extended or long-term care due to age, illness or disabilities. Also provides hospice and palliative care, including out-of-home respite care.

Provides free standing (overnight) and in-home respite services, after school respite (at the Cheektowaga Center for Learning), Saturday respite, community habilitation, and self-direction / supported living services.

Respite care opportunities for parents of persons with physical or developmental disabilities or who are medically fragile and technologically dependent.

Fundamental to the Community Independence Program is the belief that individuals with developmental disabilities can and should be given the opportunity to determine the content and character of the service they receive, identify the goals to which they aspire, and choose the individuals that would provide them with assistance in identifying and attaining their goals.

Provides periodic respite breaks for primary caregivers while disabled relative is cared for at Rising Treetops at Oakhurst. Disabled family member lives among peers and and has access to seasonal recreational activities including crafts, athletics, swimming, games and TV. Also offers overnight respite where registered families can reserve time in advance. Day respite is available to residents of Monmouth County.

Offers a skilled nursing care unit for veterans requiring extended or long-term care due to age, illness or disabilities. Also provides hospice and palliative care, including out-of-home respite care.

Offers short-term (1-30 days) respite care services for families needing time off caring for a loved one, overnight, a weekend or vacation.

Provides over night, weekday evening, and all day weekend respite for both children and adults in Thornwood, NY when a family or caregivers need time off.

Provides short term placement (up to 14 days) of children and adolescents who are in mental health crisis requiring a temporary out-of-home placement.

Nursing home, respite care available, 24 bed rehabilitation unit, nursing homes or residential health care facilities provide 24-hour nursing care and supervision outside of a hospital. Licensed and regulated by the New York State Department of Health as well as hospice programs

Provides after school recreation, respite, and transportation for learning disabled and/or developmentally disabled youth ages 12 years and up living at home. Call. Referral and appointment required.
