This program reaches out to individuals who are homeless, and or engaging in using substances. * Provides a harm reduction approach to care including providing Narcan, Fentanyl test strips, and provides resources for safe usage. The intent is for the program to wrap individuals with support and assist in getting the individual connected with services.

The CAPTAIN Street Outreach Program carries out services related to helping homeless and troubled youth leave the streets and secure appropriate, safe living arrangements, and to prevent the occurrence or reoccurrence of homelessness. Street Outreach is also in the community to help any youth who are in, or run the risk of becoming involved in a crisis. Street Outreach engage youth in their environment offering basic survival needs such as food, clothing, hygiene products, and access to emergency shelter as well as referrals, intervention, and exposure to positive adult role models. The purpose of Street Outreach is to aid youth in making positive decisions in their own lives to remain happy, healthy, and contributing members of society.

STREET OUTREACH | LONG ISLAND COALITION FOR THE HOMELESS

Provides "on the street" services to Long Island's unsheltered persons in need. Trained members offer crisis intervention and referrals for essential services, including emergency housing. Team members also distribute kits with clothes, non-perishable foods, blankets, and toiletries to individuals living on the street. The team also works with local shelters, businesses, and houses of worship to assist with resources. Please call for more information.

Health Care for Homeless Veterans - Veterans Affairs

Drop-in center for homeless vets. Provides linkage with VA services and assistance with finding housing. Works with homeless and at-risk veterans to help them overcome the barriers that may prevent them from receiving or benefiting from health services. Veterans have access to a shower in the Drop-in Center. Also offers outreach to homeless veterans in shelters, soup kitchens, and on the streets. Outreach workers help establish eligibility for VA medical service and pension, develop appropriate treatment plans, and screen for community placement. Homeless veterans eligible for VA Healthcare may call for availability and screening for VA funded emergency shelter, transitional housing, and HUD section 8 voucher program.
