Provides Nassau County residents ages 18 and older, same day access to substance use services including health screening and assessment, case management, treatment planning, medication assisted treatment, suboxone and vivitrol inductions and telemedicine. Offers referral, peer support services, outreach, educational services and individual and family counseling.

Provides services to prevent homelessness and facilitates the transition from homelessness to permanent housing. Provides direct housing search assistance, advocacy, financial assistance, case management, and job skills training. Offers bus tokens to homeless individuals for transportation to Department of Social Services.

Provides supportive services to very low income Veterans and their families who are homeless or are in danger of losing their homes. Services include outreach, case management, and assistance obtaining VA and other benefits. Provides time-limited payments to third parties, such as landlords, utility companies, moving and storage companies, and child care providers, if those payments can help Veteran families stay in or obtain permanent, sustainable housing.

Oversees the funding process for permanent housing programs that are for homeless households on Long Island. The Coalition is the central access point for housing on Long Island for persons that are homeless for the greatest lengths of time and/or have one or more disabling condition. The Coalition can assess all persons presenting as homeless in the region and refer to the best resource available. The Coalition has a homeless hotline to report street homeless persons in need of housing. Also, provides clothing, food and essentials when available. Call for further information.

Drop-in center for homeless vets. Provides linkage with VA services and assistance with finding housing. Works with homeless and at-risk veterans to help them overcome the barriers that may prevent them from receiving or benefiting from health services. Veterans have access to a shower in the Drop-in Center. Also offers outreach to homeless veterans in shelters, soup kitchens, and on the streets. Outreach workers help establish eligibility for VA medical service and pension, develop appropriate treatment plans, and screen for community placement. Homeless veterans eligible for VA Healthcare may call for availability and screening for VA funded emergency shelter, transitional housing, and HUD section 8 voucher program. The CAPTAIN Street Outreach Program carries out services related to helping homeless and troubled youth leave the streets and secure appropriate, safe living arrangements, and to prevent the occurrence or reoccurrence of homelessness. Street Outreach is also in the community to help any youth who are in, or run the risk of becoming involved in a crisis. Street Outreach engage youth in their environment offering basic survival needs such as food, clothing, hygiene products, and access to emergency shelter as well as referrals, intervention, and exposure to positive adult role models. The purpose of Street Outreach is to aid youth in making positive decisions in their own lives to remain happy, healthy, and contributing members of society.

Provides "on the street" services to Long Island's unsheltered persons in need. Trained members offer crisis intervention and referrals for essential services, including emergency housing. Team members also distribute kits with clothes, non-perishable foods, blankets, and toiletries to individuals living on the street. This program reaches out to individuals who are homeless, and or engaging in using substances. * Provides a harm reduction approach to care including providing Narcan, Fentanyl test strips, and provides resources for safe usage. The intent is for the program to wrap individuals with support and assist in getting the individual connected with services.

Provides information/literature on local support services and sexual exploitation issues to homeless youth, living on the streets of Westchester and Rockland counties.

Assist the unsheltered in Yonkers with community resources, entry point in pathway to services,. Help by transporting them to Department of Social Services, where they can apply for food stamps and medical or mental health services. They accept donations such as socks, hats, gloves, scarves. No acceptance of clothing (i.e., pants, shirts, undergarments) at this time.

Provides an outreach program with peer support for individuals at high risk for homelessness. Assists clients in making the transition from living in the shelter or the street to living in the community.

Offers an outpatient treatment-referral program designed to serve individuals between the ages of 12 and 35 who are at risk or who have been sexually exploited/human trafficked, at-risk of sexual exploitation/human trafficking, runaway or are homeless. Services provided include individual and family counseling, street outreach, GED classes, DSS advocacy, life skills education, housing assistance, and education and job placement and are tailored to meet all the needs of each individual utilizing services.
