Provides residential living in supportive apartments for intellectually disabled adults.

Offers assistance to participants to achieve goals of work, education, social connection, housing, community involvement, and health; overcoming the impact of psychiatric illness and disability that have obstructed these personal goals. Actively employs creative and expressive arts, symptom management strategies, adventure-based activities, peer support strategies, and cognitive remediation techniques to help people challenge personal barriers and rebuild their lives in recovery.

Operates residential and day programs, and Medicaid service programs serving adults with developmental disabilities. The program's goal is for each person to function at the highest level possible in the least restrictive setting. Residential programs include, Community residences (CR), Individual Residential Alternatives (IRA) and Intermediate Care Facilities (ICF) in Somers, Yorktown, Pleasantville and Ossining.

Offers community-based living for adults who have emotional disturbances but who do not require inpatient psychiatric hospitalization with the goal of transitioning them to viable lives within the community. Living arrangements are either in one of the agency run Community Residences or in a private home Family Care Residence.

* Offers individuals with differing abilities a variety of residential living options, which are comfortable, supportive, and most importantly, a secure, home environment.

* Offers day habilitation programs.

A 20-bed program providing stabilization services in a residential setting to men and women over 18, targeting individuals in need of medically supervised detoxification services; individuals experiencing mild withdrawal or significant urges for illicit substances that cannot be managed in an outpatient setting; and/or individuals who need residential care.

* Offers a broad array of services which include: Treatment Residential: Community residences. Supportive apartment programs. Single room occupancy (SRO). Crisis Respite. Supported housing. Transitional housing. Children and Youth Care management. Adult Health home care management. Forensic case management. Outreach and non-medicaid case management. Recovery support services for individuals recovering from both mental health and substance use disorders.

Provides separate adult, adolescent and family treatment facilities with specialized recovery programs on inpatient basis. Uses AA principles, includes individual and family treatment, provided on voluntary basis; networks AA contact, outpatient services, and halfway house placement for clients from outlying areas.

St. Mary's Behavioral Health Services offers information on adult and children's psychiatric evaluations. The St. Mary's Children's Mental Health Clinic provides outpatient mental health services for seriously emotionally disturbed children up to 18 years of age and their families who reside in Fulton, Montgomery and Hamilton counties. Limited evening and and Saturday appointments may be available upon request. St. Mary's Emergency Services provides 24-hour-contact with mental health services including emergency telephone counseling. Other services include in-person evaluations, intervention and assessment.

Substance Abuse Treatment CenterNarcan training and distribution

Provides a care coordination program that assists individuals that are residing in a community setting or are transitioning to live in a community setting. Services include behavioral, educational, and social health support, transportation, and housing support. On-call services are available 24/7.

Residential and home services to adults and children with developmental disabilities and/or mental illness, including respite services, supervised congregate care, and family care. Transportation is available for Family Care and IRA. Supportive Apartments are available to those individuals with developmental disabilities ages 18+.

Committed to assisting clientele to live meaningful, productive lives by providing flexible supports in a person-centered atmosphere. Residents of all programs learn the skills necessary to live independently. Treatment apartments are certified by the New York State Office of Mental Health and provide consumers with the highest level of independence. Site is staffed 24 hours a day, 7 days a week. 31 one-bedroom apartments are available, two of which are handicap accessible.

Some apartments are located at a main complex, where staffing offices are located for more intensive support and structure. Additional apartments are available throughout the Niagara Falls community for those needing less support. Double units are available on-site. Rockland Hospital Guild provides residential programs for adults with hearing impairments and mental illness.

Community, group and individual housing services for adults of all ages with an emphasis on personal choice.

Offers full range of residential alternatives for intellectually and/or developmentally disabled adults.

Provides adult residential services to developmentally disabled individuals, ages 21 and over who exhibit challenging behaviors. These homes are located at multiple locations in Suffolk and Nassau Counties.

Provides supervised living programs for 32 residents on the grounds of Pilgrim Psychiatric Center and 44 residents in four houses in the community. Vocational training, volunteer placement, employment, day treatment or continuing education are encouraged. Emphasis is on ADL and enhancing socialization skills. Specialized programs are also available for MICA, and low motivated residents.

Provides a 24 hour supervised residence for several adults with severe developmental disabilities. Services include medical and psychiatric care, basic living skills training, case management and physical, occupational and speech therapy.

OMH Certified Treatment Apartment Program. A staff person visits these apartments 3-5 times a week, relevant to provision of community rehabilitation skills. Staff are available 24 hours a day, 7 days a week.
