Care coordination is a person-centered planning process that takes place between a qualified case manager/care coordinator, the applicant, and, often, members of the applicant’s informal support network. Care coordination begins with a comprehensive assessment that identifies an individual’s needs, abilities, preferences and available resources. Next, a care plan is developed based on the findings of the assessment. The care plan is a coordinated package of services and supports derived from multiple sources to best meet the applicant’s needs.
