Program designed to help seniors remain safely in the community and also to help them to flourish by remaining connected with medical, behavioral health and non-medical supports in the community. Three program components are as follows: Aging Services access can screen for aging needs that may place the older adult’s ability to remain in the community in jeopardy and can assist with linkages to needed services for nutritional support, personal care, transportation, financial stability, and increasing socialization. Behavioral Health access screens for behavioral health needs and can help individuals get connected to needed mental health or substance abuse services. Mobile Outreach and off-site services include services provided by the Older Adult Clinical Specialist who can provide short-term in-home therapeutic assistance as well as assisting those individuals who are not connected to needed services or who may have difficulty accessing services

Family Health Network's Cincinnatus Health Center provides comprehensive primary medical care services directly and by referral.Services offered by this Health Center include: family practice/general practice occupational health services 340B pharmacy services insurance enrollment

Free and confidential counseling for Broome County youth (8-21 yrs). Parental permission not required, but family involvement is encouraged. Provides assessment, short-term counseling, referral, psycho- educational group counseling (anger management, self- esteem, social skills, and parenting) and coordination of services to youth and their families.

Provides a short-term, medically supervised outpatient services for adults 18 years of age and older. Offers an alternative for a person who is transitioning from inpatient to outpatient care.

Free, voluntary and confidential emotional and behavioral health screening for youth to prevent small problems from getting bigger. Parents fill out a checklist and send it to an agency screener. Screener contacts parents about results and discusses options and resources in the community.

Offers comprehensive outpatient service for children and adolescents. Designed to meet the needs of children who have severe emotional and/or psychiatric illness.

Provides a free anonymous mental health and alcohol education screening program for military personnel and their families, in all branches, including National Guard and Reserve. The self-assessment is a series of questions that help create a picture of a person is feeling and whether they could benefit from further evaluation and treatment.

Mental Health Services for residents of Otsego County

Provides outpatient mental health services for individuals ages 18 years and older.

Serves youth (13-17 yrs) who are in situational crisis caused by an event in the adolescent's environment, such as family problems, loss of relationships, neglect, abuse, school problems, or homelessness.

Provides up to 12 months of intensive and comprehensive evidence-based treatment for youth and young adults who struggle with thoughts of suicide.

Helps individuals and families resolve issues in accessing substance use disorder and mental health services.
