Provides treatment to individuals ages 3 years old to 21 most commonly, however, can accommodate treating individuals older than 21 as well as the family members of the children serve. We have a staff of licensed mastered level counselors, a medical team that includes: a child and adolescent psychiatrist, nurse practitioners and a registered nurse. We provide individual and family counseling (mental health and drug/alcohol), Case Management, Health Monitoring and Advocacy with a strong emphasis on family focused treatment.

Offers fully integrated mental health and substance use disorder treatment and services for adolescents and adults. Services include: - Counseling and medication treatment services for a variety of mental health and substance use disorders - Psychiatric evaluations - Substance use disorder medication assisted treatment - Health care screening and monitoring for overall health - Skills building - Education assistance and vocational services - Targeted case management and peer services

Facilitates a smooth transition from the hospital to home for Nassau County children under 18 years of age with mental illness. Provides crisis assessment, individualized planning and family needs assessments. Offers family support by a treatment team to ensure adequate discharge planning and access to community-based services to promote long term symptom management and recovery.

Coordinates enhanced mental health services for children and youth who are at risk of or have been diagnosed with a serious emotional disturbance (SED) and their families in Suffolk County. The CSPOA offers a centralized, coordinated referral process using a universal referral form and represents a consensus among community-based and residential programs to ensure appropriate service delivery to the highest needs youth. The program serves Suffolk County residents, ages 5 to 21 years, and their families. All C-SPOA programs are voluntary for both the youth and their family. Call for detailed information.

Guests self-refer, come and go as they desire, and have 24/7 access to peer counseling/support and education to assist through troubling times. Mobile response team also provided for Dutchess county. Hospital Diversion Houses have helped many people to regain wellness while maintaining independence in the community.

Serves individuals (age 18+) who have Medicaid and meet the criteria for Health Home services. Will assist, support and monitor with the client all major systems. Designed to promote the indivduals optimal health, reduce unnecessary emergency room visits and inpatient stays while enhancing the efficacy of all systems with whom the individual is engaged. This will include but not be limited to; living arrangement, physical and behavioral health, economics, entitlements, vocational training and social, family, self-care and legal issues.

Provides a court ordered service for the adult clients who are non-compliant with psychiatric medication and as a result decompensate and are deemed unable to live safely in the community. The initial AOT order lasts for six months and if renewed the order could last for another year. Mandated services include, but are not limited to, medication management and case management or ACT. The Nassau County Department of Mental Health is required to maintain oversight responsibility for all clients on AOT orders. Call for additional information and inquiry.
