Provides a transition management program for individuals with severe mental illness who are ready to leave a residential treatment program, which offers support for improved community service linkages and timely filing of Medicaid applications or other benefits, and help with obtaining post-release services.

Federation of organizations offers over 750 permanent, safe and affordable residential placements in our supportive scattered-site Housing program. In this program, individuals with disabilities live in apartments or houses throughout Nassau, Suffolk, Queens, Brooklyn and the Bronx. Caring people open their homes to adults that are mentally or physically impaired who do not require the skilled care of a nursing facility. A request to become a Family-Type Homes for Adults operator or to refer an interested adult may be made directly to the Office of Adult Services.
