Provides a family living situation for adults who need supervision and care. Caring people open their homes to adults that are mentally or physically impaired who do not require the skilled care of a nursing facility. A request to become a Family-Type Homes for Adults operator or to refer an interested adult may be made directly to the Office of Adult Services.

PATHWAYS - SUPERVISED PROGRAM | TRANSITIONAL SERVICES OF NEW YORK FOR LONG ISLAND

Provides supervised living programs for 32 residents on the grounds of Pilgrim Psychiatric Center and 44 residents in four houses in the community. Vocational training, volunteer placement, employment, day treatment or continuing education are encouraged. Emphasis is on ADL and enhancing socialization skills. Specialized programs are also available for MICA, and low motivated residents.

We provide specialized supportive scattered-site housing placements that target vulnerable populations such as veterans, shelter-homeless individuals transitioning out of adult homes, state psychiatric centers, prisons and local hospitals.
