A program, in collaboration with the Mamaroneck school system, to educate school students on the dangers and impact of alcohol and drug use and addiction.

Educates youth about the dangers of alcohol and drug abuse, and ways in which to resist becoming involved with them, and helping those who are struggling with addiction.

Description: Federally Qualified Health Center (FQHC) offering pediatrics, internal medicine/family medicine, OB/GYN, pediatric and adult dental services, podiatry, social work, nutritional counseling, medications for opioid use disorder, vision care, and pharmacy with home delivery. Serves all populations with special focus on the medically under-insured, uninsured, and those experiencing homelessness. Also provides onsite lab, ultrasound, STD testing, COVID-19 testing and vaccines, health education/groups, blood pressure screening, and free walk-in pregnancy testing. Provides transportation assistance to help clients get to appointments and referral assistance. Participates in 340B drug discount program and has onsite enrollers for the New York State of Health insurance marketplace. Hospital affiliations: Sisters Hospital and the John R. Provides care coordination, specialty and primary health care, HIV/AIDS specialty care, LGBTQ+ specialty care, STI/HCV testing, syringe exchange, buprenorphine program, and harm reduction program. Also provides linkage to mental health services, substance use counseling, housing assistance, nutrition assistance, and transportation assistance.

Outpatient clinic services include Adult Services, Alcohol and Substance Use Services, and Child and Adolescent Services. Available services include individual, family, and group therapy. Evidence-based practices are the treatment modalities of choice. People receiving services are encouraged to have their family or significant other participate as well. People receiving services are involved in the development of their treatment plan, as are their family/caregivers to the extent the person desires their involvement or when they are legal guardians. Services can be provided through telehealth as well as in-person. These options are provided in order to help facilitate access to appointments, deal with emergency appointments as needed, and to improve engagement. Sessions are conducted through secure technology platforms, which have been approved by New York State.

Provides a bi-lingual, six session program for children ages 8-11 to teach them how to better deal with their asthma.

Provides a comprehensive outpatient treatment program for individuals struggling with alcohol or drug addiction.

Provides a free anonymous mental health and alcohol education screening program for military personnel and their families, in all branches, including National Guard and Reserve. The self-assessment is a series of questions that help create a picture of a person is feeling and whether they could benefit from further evaluation and treatment.

The North Country Chronic Disease Coalition’s goal is for each Health System Service Area to have developed and started implementation of a Community-Clinical action plan. These plans will create and/or increase awareness of health-promoting resources that providers can refer patients to for community-based chronic disease risk reduction activities and support.

Residential reintegration program for homeless men in recovery from substance abuse that encourages connections to volunteer, educational, and employment opportunities. Offers counseling, meals, recreation, daily support, and self-help meetings. Program employs client-centered cognitive behavioral and dialectical behavioral therapies, along with motivational interviewing to help residents learn to cope with life experiences without relying on substance use. Utilizes trauma-informed care practices that seek to address each individual's past history or trauma and its effect on their lives.

Persons who are age 18 years or older may legally obtain and possess hypodermic needles and syringes through ESAP without a medical prescription. Licensed pharmacies, health care facilities, and health care practitioners who can otherwise prescribe hypodermic needles or syringes may register with the New York State Department of Health to sell or furnish up to 10 hypodermic needles or syringes to persons 18 years of age or older. Used needles or syringes can be safely and anonymously disposed of at dedicated disposal kiosks throughout Erie County; see separate listing, Needle Disposal - ECDOH, for details. The Erie County Department of Health (ECDOH) is a distribution site for limited free medical supplies, such as syringes, band aids, cotton, alcohol wipes, etc. for Erie County residents.

Provides alcohol and substance use prevention / early intervention programming for schools. Offers alternative activities such as coordinating youth projects, community education on alcohol, substances, and gambling prevention, and family intervention services.

Provides the following services, conforming to a harm reduction model of care: Substance Use Counseling: Creates an action plan to support recovery and provides linkage to providers that will help achieve recovery goals. Narcan Overdose Reversal Training: Takes less than 10 minutes and provides a free Narcan kit. Syringe Exchange Program: Provides an exchange of used syringes for clean ones to people who inject substances like insulin, steroids, heroin, and cocaine. Individual & Group Support: Provides linkage to treatment and other services, along with overdose follow-up and prevention.

Outpatient drug and alcohol treatment facility licensed by Office of Alcohol and Substance Abuse, a division of Chautauqua County Department of Mental Hygiene. Provides assessments for adults, youth, and families. Treatment options include individual and group therapy.
