Through community partnerships this program places a team of youth outreach workers in the community as a resource for adolescents and their parents.

Informs and assist members of the armed forces, veterans, their dependents and survivors in relation to (1) matters pertaining to education, training and retraining services and facilities, (2) health, medical and rehabilitation services and facilities, (3) provisions of federal, state and local laws and regulations affording special rights and privileges to members of the armed forces, veterans and their families, (4) employment and re-employment services, and other matters of similar related or appropriate nature. Provides outreach and assistance to people in need of food, entitlement programs, information, and referrals. The van visits user friendly locations, libraries, homeless enclaves, parks, community events, and shopping center parking lots. Includes an on board food pantry with basic food staples.

Offers Suffolk County adolescents, seniors and adults struggling with alcohol/drug abuse and their family members access to mobile recovery services. Provides comprehensive substance abuse and mental health assessments, diagnosis, treatment referrals, medication and case care management, outside a clinic setting. Provides assistance with medical, housing, financial, employment, legal and family issues. Offers individual, family and group therapy, recovery support with peer advocates, telepractice and medication assisted treatment at a mobile OASAS designated Center of Treatment Innovation.

Hosts an annual conference for families and service providers, offering trainings and educational workshops. Attends various conferences and other events exhibiting and distributing information.

ADRC plays an important role in educating Long Island’s caregivers. Both professional caregivers who work in assisted living facilities or nursing homes, and individual family members who have a loved one with Alzheimer’s, benefit from increased knowledge and practical training exercises.

Offers Long Island students in grades 4-8, who are in need of positive role models, supervised one-to-one and group mentoring relationships. Weekly one hour meetings allow time for mentors and students to interact, collaborate and socialize .

Counselors offer individual counseling, facilitate survivor support groups, accompany victims to court, advocate for client needs and work with families to prepare for the transition to a violence-free home. Safety planning is conducted throughout the counseling process.

Provides networking and information exchange opportunities among professionals serving seniors, ongoing education opportunities, advocacy for seniors and community outreach. Offers over 26 outreach programs to the local community. Ministries include youth outreach for children 8-16 years of age, academic enhancement which includes tutorial program, homework assistance, computer and study workshops.

Provides support and information to families with special needs children. Also matches parents, by request, with a trained support parent who has a child with a similar disability, chronic illness, or health care concern to help with parenting issues. This helps to build a support network and connect to tools and resources in the community. Also offers speakers to educate health care professionals, teachers, and others who care for children with special needs.

Proyecto Faro builds community and organizes Rockland immigrants by providing monthly legal clinics, food program and financial support.

Independent Living provides a bridge between the Deaf and Hard-of-Hearing cultures in Orange, Ulster, Sullivan and Dutchess Counties, this program is actively engaged in community outreach and education, employment, advocacy, service coordination, counseling and technical support.

Provides prevention and youth development programs, education and referral to general youth populations, as well as those identified as high risk.

An outreach program to help improve and maintain personal health and environmental living conditions. The goal of the program is to promote healthy neighborhoods by keeping residents aware of the variety of medical, social and environmental services available to them locally. Outreach staff visit residents, at the residents request, an inspection of their home is also performed to be sure that no environmental health hazards or problems exist. If environmental health hazards or problems do exist, our staff will direct them to the agency best able to help.

Provides people in the early stages of Alzheimer's disease/dementia and their caregivers education programs and social activities that improve relationships, increase confidence and decrease emotional triggers. Programs vary, but can include; early-stage educational programs, support groups, meetups, and social engagement programs such as Memory Cafes and exercise programs.

We host monthly Caring for Two class on the first Thursday of every month from 12-1:30pm, where a team of lactation professional, WIC counselors, and pediatric residents (MDs) discuss topics ranging from jaundice to nutrition, to breastfeeding support.

Support and education available for caregivers of people with Alzheimer's disease and other dementias. Education programs, support groups, respite care, outreach, in-home assessment and care consultations are available. This grant targets underserved communities in Nassau and Suffolk Counties. Bilingual services in Spanish are available.

Conduct education, outreach, engagement, referral, enrollment and follow-up activities that will increase access to high-quality HIV health care.

Encourages youth to establish a good work ethic through employment and workshops. Promotes punctuality, teamwork, how to follow rules and regulations, as well as respect of self, co-workers and supervisors. At- risk youth are selected between the ages of 14-18. Interviews are held in May and orientation in June. Employment is seven weeks, Monday-Friday. Worksites are established within the community at local churches, schools, agencies, parks and beaches. Workshops are given throughout the program in areas such as Banking, Job Attitudes, Career Goals, Job Interviewing and Resume Writing. In partnership with the Alzheimer’s Association of Long Island, provides educational seminars for community members and support groups for families and caregivers. In addition, provides an Aging in Place Center, a Memory Cafe, and Adult Day Care programs.

Provides one of the most vital units throughout the Sheriff’s office. Throughout the year, Community Relations Unit visits each Suffolk County School District as well as multiple clubs and various organizations. Its main goal is to bridge the gap between Law Enforcement and the members of the Community. CRU is responsible for implementing community outreach and school-based programs, organizing events, and promoting the Sheriff’s mission.
