Conducts telephone screenings to determine Cortland County residents' eligibility for the SNAP (food stamps) program.After screening, the outreach specialist recommends the best application process for the client, including face-to-face meetings, apply by mail, online application or direct referral to the Department of Social Services with a list of required documents.

Identifies older individuals who may have significant visual impairments, hearing loss, and deafness and assists them with applying for services to which they are entitled.

The mission of Charles Evans Center is to improve the health and quality of life in underserved populations including those with autism and developmental disabilities.

Informs and assist members of the armed forces, veterans, their dependents and survivors in relation to (1) matters pertaining to education, training and retraining services and facilities, (2) health, medical and rehabilitation services and facilities, (3) provisions of federal, state and local laws and regulations affording special rights and privileges to members of the armed forces, veterans and their families, (4) employment and re-employment services, and other matters of similar related or appropriate nature. Serves as a resource center to assist individuals using the Mary Brennan soup kitchen in obtaining needed services and assistance in becoming self-sufficient. Provides referrals to Social Service agencies including employment, housing, and immigration. Offers workshops for self-improvement including English classes, cooking demonstrations, money management, computer training and health screenings. Provides assistance with clothing and housewares through its boutique.

Through community partnerships this program places a team of youth outreach workers in the community as a resource for adolescents and their parents.

Coordinates outpatient services, arranges for patients to participate in clinical trials, and provides counseling, testing, and community education.

Provides home nursing, home health aides and personal care services for those who require long term care due to age, illness, injury or disability. Also provides prevention and outreach programs for those who are diagnosed with AIDS/HIV.

Offers Community Outreach/Injury Prevention Programs. Sample programs include: Concussion/TBI management for student athlete/parent/coach/school faculty sports safety information sessions; Preventing Falls in Older Adults; Tai Chi for Arthritis and Fall Prevention; Stepping On, Trauma Talk and Stop the Bleed. Provides Independent Living Services by culturally sensitive, bi-lingual staff members who focus on issues directly related to and which impact the disabled Hispanic Community. Provides services such as peer counseling, advocacy, housing referrals, Consumer Directed Personal Assistance Program (CDPAP), limited transportation, equipment loan, etc. to the Hispanic community in Nassau County. Creative arts workshops are provided. Appointment required.

MICHC will provide education, outreach, home visitation, referrals and follow-up to improve the general health and well-being of women and infants, with a focus on family planning and self-sufficiency. The program will engage women and link families to health and other supportive services in the targeted communities of Newburgh, Middletown and Port Jervis.

Available for Health Fairs, Awareness Days, Community Events, and Facilitator/Staff Trainings.

Offers education and public awareness about AIDS/HIV and sexually transmitted disease, designed specifically for teenagers and young adults, also give information and referrals to resources such as HIV testing and counseling.
