Provides habilitation services within a person's private home or apartment. Hours provided by staff vary based on a person's needs. The main goal of this program is to increase independence while providing support to the person and the family. People are encouraged to strive to live independently in the community with minimal staff support.

Affiliated with Stony Brook University Medical Center. Provides consultation services in a variety of specialized areas, including neurosurgery, pediatric and adult orthopedics, radiology, pediatric surgery, pediatric and adult urology, as well as general surgery. Also offers anti-partum testing, free prostate cancer screening, hi-risk pregnancy consultations, and midwives. Out patient services are performed on site, and patients requiring admission to University Medical Center for more complex procedures may receive all pre- and post-operative care at the center.

AIDS SERVICES | NASSAU COUNTY DEPARTMENT OF SOCIAL SERVICES

Ensures that the people who have been diagnosed as being HIV or having AIDS receive all the services and benefits for which they are entitled from the Department of Social Services and many public and private helping agencies. The Department has a dedicated HIV/Aids caseworker who assists visitors. For individuals who come to the building, the caseworker meets face to face and offers services available both at DSS and in the community. Call for detailed information.

Provides coordination of services for legally blind veterans and their families. Duties include providing and/or arranging for the provision of appropriate treatment modalities to enhance a blinded veteran's functioning level. Other duties include identifying new cases of blindness, providing professional counseling, resolving problems, arranging annual healthcare reviews and conducting education programs relating to blindness.
