AIDS SERVICES | NASSAU COUNTY DEPARTMENT OF SOCIAL SERVICES

Ensures that the people who have been diagnosed as being HIV or having AIDS receive all the services and benefits for which they are entitled from the Department of Social Services and many public and private helping agencies. The Department has a dedicated HIV/Aids caseworker who assists visitors. For individuals who come to the building, the caseworker meets face to face and offers services available both at DSS and in the community. Call for detailed information.

ST. MARY'S HOME CARE ASTHMA MANAGEMENT PROGRAM | ST. MARY'S HOME CARE ASTHMA MANAGEMENT PROGRAM

Offers a team approach (nurses, social workers, therapists and nutritionists) to provide patient/family centered asthma education and manage asthma care at home. Services include medication management, environmental and lifestyle assessment, emergency management and health monitoring through a telehealth program. Works collaboratively with patient's physician to follow treatment plans and manage asthma effectively. Communicates with school health staff and educators as needed to support children attending school.

PATIENT SUPPORT GROUPS | LEUKEMIA AND LYMPHOMA SOCIETY

Teaches beauty techniques to female cancer patients to help them manage the appearance-related side effects of cancer treatment. SPARC-RIVERHEAD | SPARC-RIVERHEAD

Offers short-term intensive interventions for people living with HIV who are either newly diagnosed or previously diagnosed but not in care in order to successfully link them with medical care and case management services. Also offers free rapid HIV testing. Respite and transportation services are available for clients of SPARC Consortium member agencies participating in each program. SPARC's Women's Care Coordinators work to ensure that clints are engaged and retained in care and are linked to other needed services in the community. SPARC's nutritionist offers comprehensive assessments, individual and group education.

Family Advocacy Counseling and Training Services Program | Brain Injury Association of New York State - Finger Lakes Area

Provides linkages for individuals with brain injuries received prior to age 22 and their families with community-based services and supports including understanding and navigating the current system of available services, accessing Medicaid Waiver Programs if qualified, training family members, service providers, and educators to understand the impact of brain injury and brain injury prevention, and making connections to community support groups about living with a brain injury or caring for an individual with a brain injury. Assistance is also provided to understanding the local school system’s special education process. This program is funded by the Office of Mental Retardation and Development Disabilities (OMRDD).
