Offers Home Health Aids (HHA'S) Personal Care Aids (PCA'S), and companion care services throughout Nassau County. Services are provided on an hourly basis through private pay, long term care insurance, Office of the Aging, Medicaid, Workers Compensation, Managed Long Term Care (MLTC), and Mainstream Managed Care (MCO) programs.

Provides in-home skilled nursing care, rehabilitation therapies, medical social workers,home health aides, intravenous therapy, medical supplies/equipment, laboratory services and pharmacy supplies. Also offers Mom and Me Care to new mothers and newborns. Home care is provided by nursing staff from the maternity department.

Services include disease, pain and medication monitoring and management, live-in companion service, nutritional counseling and assistance, occupational and physical therapy, respite care for caregivers, socialization, personal care and assistance, end-of-life care and private duty nursing. Frozen prepared meals are available for weekends if there is a demonstrated need. Also offers congregate dining five days a week/one meal a day. Transportation is available.
