PROJECT VETERANS' INDEPENDENCE | PROJECT VETERANS' INDEPENDENCE

Project Veterans' Independence is a Catholic Charities program partnership with the Department of Housing and Urban Development (HUD). Our goal is to place homeless veterans into permanent housing and help them live independently within their communities while offering counseling and access to support services. Supportive case management, peer-to-peer support groups and crisis intervention services are available to all clients.

NATIONAL CALL CENTER FOR HOMELESS VETERANS | NATIONAL CALL CENTER FOR HOMELESS VETERANS

Offers a hotline to ensure that homeless Veterans or Veterans at-risk for homelessness have free, 24/7 access to trained counselors. The hotline is intended to assist homeless Veterans and their families, VA Medical Centers, federal, state and local partners, community agencies, service providers and others in the community.

SUPPORTIVE SERVICES FOR VETERAN FAMILIES MTA CONTRACT | SERVICES FOR THE UNDERSERVED - SUS

Provides outreach, food, clothing and housing. Also provides assistance in completing applications and transportation to placements, individualized assessment for crisis support, transitional care coordination, short term case management, transportation to Social Services, advocacy assistance and referrals.

SUPPORTED HOUSING | SUPPORTED HOUSING

Provides housing and supportive services aimed at assisting clients who have a psychiatric disability to maintain themselves in the community. Serves Nassau County residents 18 and over with a primary psychiatric diagnosis. For certain locations, individuals must also meet HUD requirements for homelessness. Referral required. OASAS program-primary substance abuse diagnosis and subsequent hospitalization in addition to homelessness.

BREAKTHROUGH | MERCY HAVEN INC - MAIN OFFICE

Offers a community based outreach and educational program targeting individuals who are currently homeless or have experienced homelessness. Provides individuals with skills and strategies to develop self-empowerment, assists with goal setting and provides local mentors. Also offers outreach services to families of program participants.

Provides 24 hour crisis intervention and referrals for youth-at-risk through telephone switchboards, as well as advocacy and educational services. Assists with conference calls and message delivery to parents of runaways, and to shelters and other agencies to confirm appropriate and available services.
