SUPPORTED HOUSING | SUPPORTED HOUSING

Provides housing and supportive services aimed at assisting clients who have a psychiatric disability to maintain themselves in the community. Serves Nassau County residents 18 and over with a primary psychiatric diagnosis. For certain locations, individuals must also meet HUD requirements for homelessness. Referral required. OASAS program-primary substance abuse diagnosis and subsequent hospitalization in addition to homelessness.

Offers a hotline to ensure that homeless Veterans or Veterans at-risk for homelessness have free, 24/7 access to trained counselors. Program is available to those individuals who require more support, but no longer need a community residence. Staff members visit on an as needed basis and assist in medication monitoring and additional services. Must meet HUD criteria.
