Provides housing and supportive services aimed at assisting clients who have a psychiatric disability to maintain themselves in the community. Serves Nassau County residents 18 and over with a primary psychiatric diagnosis. For certain locations, individuals must also meet HUD requirements for homelessness. Referral required. OASAS program-primary substance abuse diagnosis and subsequent hospitalization in addition to homelessness.

Provides 31 units of permanent housing and supportive services for high need homeless, and psychiatrically disabled adults. Offers rehabilitation services, teaching of coping skills and crisis intervention. Offers staff assistance 24 hours a day.

Program is available to those individuals who require more support, but no longer need a community residence. Staff members visit on an as needed basis and assist in medication monitoring and additional services. Must meet HUD criteria.
