Provides information, referral, and advocacy for children birth to 21 years who have, or are suspected of having a serious or chronic physical, developmental, behavioral, and/or emotional condition and who require health or related services of a type or amount beyond that typically required of children. Provides case management services for families to advocate with health care providers, obtain information on programs available, advocate with school districts for related or special services, make referrals for special health care or related services, and obtain family support services.

Offers information and assistance, education, and screening for older adults ages 60 and older, adults and children with disabilities, as well as family members, caregivers, and professionals.

A comprehensive health center offering primary care (assessment, treatment, and referral), dental clinic, health education, and diabetes education. Tuscarora Community Health Worker Program provides home-based case management services to participating women and their children and helps them overcome barriers to care. Workers help identify complicating factors that make it difficult for clients to access the care they need; assist in enrolling pregnant women into prenatal care and children into primary care; assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus; provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence, and substance abuse; provide referrals to food pantries, housing assistance, educational programs, childcare, and other services; and provide support and help to overcome the barriers that prevent families from receiving a healthy start in life. Offers comprehensive care to women of all ages. Services include obstetrics and gynecology, substance use and addiction treatment, maternity care, nutritional needs, joint replacement, and treatment for heart disease, breast abnormalities, diabetes, lymphadema, osteoporosis, and urinary incontinence.

A CCBHC is a community-based clinic that offers integrated mental health and substance use care, along with crisis services and care coordination. Instead of navigating multiple systems, individuals can receive support through a single, connected team. Provides comprehensive mental health and substance use services for children, adolescents, adults, and families, all in one place. Services include but are not limited to:

• Individual, family, and group counseling
• Psychiatric evaluation and medication management
• Substance use treatment, including Medication-Assisted Treatment (MAT)
• Integrated mental health and substance use care
• Crisis support, including walk-in and same-day services when available
• Care coordination and peer support
• Trauma-informed, evidence-based therapies

Aging and Disability Resource Center for Erie County serving as a "one-stop" for help navigating the Long Term Care Systems in Erie County. A comprehensive screening process by Information and Assistance Specialists provides personalized options counseling to help consumers make informed decisions and assist in accessing available Long Term Services and Support options. Information and assistance provided to older population, disabled individuals of all ages, and caregivers. Information and assistance is available in various ways including telephone access; face-to-face meetings at the office, in the community, or in the home; and via email. Also provides an online resource directory of providers of long term services and supports, information about the different types of such services, and contact information for the local programs; directory available at www.nyconnects.ny.gov. Serves as the point of contact for several other programs run through the Department of Senior Services, such as the Senior Farmers' Market Nutrition Program, which provides low-income seniors with coupons that can be exchanged for eligible foods (fruits, vegetables, honey, and fresh-cut herbs) at farmers' markets, roadside stands, and community supported agriculture (CSA) programs. A 25-bed residential treatment facility located on the Horizon Village campus that provides specialized behavioral treatment services for veterans, first-responders, and civilians with PTSD and addiction disorders. Veterans are linked with Vet One Stop and Compeer services. Clinicians are trained in EMDR to address PTSD symptoms. Provides medication assisted treatment: Suboxone, Sublocade, and Vivitrol. ##Veteran##

Provides comprehensive treatment, recovery, and support services for individuals and families impacted by mental health and substance use challenges. Services offered include individual and group counseling, medical and psychiatric care, family support coordination, addiction medicine consultations, opiate overdose prevention training, recovery coaching and peer support services, care coordination, community outreach and education, harm reduction services, a therapy dog program, and more. Federally Qualified Health Center offering OB/GYN, social work, and nutritional counseling. Serves all populations with special focuses on the medically under-insured, uninsured, and those experiencing homelessness. Also provides free walk-in pregnancy testing, onsite lab, ultrasound, STD testing, health education/groups, blood pressure screening, transportation, and referral assistance. A home visiting program for Chautauqua County mothers and their children from pregnancy to age 2. Helps families to achieve healthy outcomes through an evidence-based home visiting curriculum, developmental screenings for children, screenings for mothers, and resources and referrals to address social determinants of health. Provides the following services:

• Education on health topics and private supportive visits in the comfort of the family's home
• Linkage with nutrition programs and breastfeeding supports
• Assistance finding a primary care doctor, dentist, physicians who can address any current health concerns, and medical/health insurance
• Assistance applying for WIC
• Assistance obtaining family planning and birth control
• Linkage to other services in the community
• Assistance scheduling appointments for mothers and children
• Assistance with transportation needs and translation
