Provides comprehensive outpatient mental health services which include psychiatric treatment, medication management, psychiatric evaluation, rehabilitation, support, self-help, crisis, health education, peer run groups, and vocational counseling.

Provides free civil legal services for individuals who are HIV positive or who have an HIV positive family member, including but not limited to: end of life planning, care and custody planning, child support, custody, visitation, family offenses, divorce, discrimination, bankruptcy, housing problems, and name change.

Drop-in center for homeless vets. Provides linkage with VA services and assistance with finding housing. Works with homeless and at-risk veterans to help them overcome the barriers that may prevent them from receiving or benefiting from health services. Veterans have access to a shower in the Drop-in Center. Also offers outreach to homeless veterans in shelters, soup kitchens, and on the streets. Outreach workers help establish eligibility for VA medical service and pension, develop appropriate treatment plans, and screen for community placement. Homeless veterans eligible for VA Healthcare may call for availability and screening for VA funded emergency shelter, transitional housing, and HUD section 8 voucher program. Provides information to caregivers on what services are available for a family member or friend and how to obtain them. Staff complete Options Counseling to link caregivers with additional information, supports, and programs throughout Erie County so they can make an informed decision that aligns with them and their loved ones preferences, strengths, values and needs. Services include: -Social Adult Day Services (SADS): Linkage and funding to contracted SADS programs to provide the caregiver a well needed break and to provide enriching activities for their loved one. -Overnight Respite: Funding for overnight care (generally 5 nights) for a loved one in a skilled nursing facility or assisted living so the caregiver can go on a vacation. -Caregiver Counseling: Linkage and funding for six (6) sessions with a professional support counselor. -Alzheimer's Disease Caregiver Support Initiative: Partnership with the Alzheimer’s Association to provide supportive services to caregivers who are caring for someone with Alzheimer’s Disease or a memory loss diagnosis. Services include care consultation, short-term respite, linkage to safety devices and legal services. -Caregiver Community Information and Resource Packet: Information on respite and support services offered by agencies throughout Erie County. -Kinship Caregivers: Linkage to community and legal supports for older adults caring for a grandchild or other child that is not their own. *Services limited to grant funds available and if eligibility requirements met.

Supervises individuals placed on probation; completes pre-sentence investigations for the courts; assists individuals in accessing Family Court/custody issues; administers juvenile delinquency and persons in need of supervision (JD/PINS) services; screens pretrial defendants for release from jail. ##REENTRY##

Services include pre-health maintenance, rapid HIV test (15 minutes), tuberculosis testing, COVID-19 testing, complete lab services, GYN services, social work services, short-term counseling, full dental care, personalized education about chronic illnesses, and smoking cessation. Family Planning Benefit Program is a direct Medicaid public health insurance program for individuals and families who need family planning services, with no premiums or co-pays for services. Program is intended to increase access to confidential family planning services and to enable individuals of any age and sex to prevent or reduce the incidence of unintended pregnancies. Provides list of registered and licensed child care providers for parents who are looking for child care. Assists individuals who want to become registered, licensed, or legally exempt providers in child care. Provides child care training for child care providers.

An outreach program that provides services to pregnant and/or parenting teens (mothers and fathers, including their caregivers) attending Buffalo Public Schools. Coordinates support services by forming links between families, community agencies, and schools. Also provides resource services and information that supports the physical, social, and emotional development of both the student and the child. Services provided include:

• Resources pertaining to pregnancy, child rearing, and home life, including referrals to community organizations that specialize in working with pregnant and parenting teens
• Assistance with completing and submitting important paperwork, such as for family court, paternity, child support or day care vouchers, and BPS pre-school applications
• Assistance with scheduling and keeping medical appointments
• Help with academic needs, including home instruction and tutoring programs
• Monitoring of academic and career goal paths and achievement
• School and home visitation
• Provision of new and gently used baby items

A process designed to help families organize services from several agencies, as well as family and friends. Goal is to help families receive the supports and services necessary to prevent the out of home placement of children. Process helps the family identify and obtain needed services and helps the service providers become more in touch with the goals of the family.

Focuses on in-school "at-risk" youth, ages 14-21, who reside in the City of Buffalo, attend Buffalo Public Schools, and are Temporary Assistance for Needy Families (TANF) eligible. "At-Risk" refers to youth who are under-achieving and are potential dropouts or need extra support due to environmental, economic, personal, or family circumstances. Program services include academic support to ensure that youth receive education, social development and life skills training to ensure they are equipped for economic self-reliance and citizenship, and career exploration to help guide students along the pathway to their desired career.

Day program for individuals with intellectual or developmental disabilities who want to be more involved in their community, but for whom employment is not appropriate. Services include community outings and interaction with others outside of their family.

Provides assistance with filing Family Offense Petitions to obtain an Order of Protection, assistance filing Custody and Support Petitions, a 24-hour hotline, accompaniment to court, law enforcement agencies, or hospital, personal advocacy, safe house, safety planning, support groups, individual counseling, and information and referral to community resources. Also offers safe house temporarily shelter for individuals in a domestic violence situation.

A network of over 85 housing counseling and legal services organizations across the state that provide free, qualified mortgage assistance relief services to homeowners. HOPP representatives can connect New Yorkers to high-quality counseling or legal services all over New York State. The HOPP Program is designed to ensure that no family in New York State ever loses their home because they do not have access to a lawyer or qualified housing counselor. Services include:

• Assistance with applying for a loan modification, or other types of loss mitigation
• Direct representation in court proceedings, including settlement conferences
• Homeownership counseling

Operates HomeownerHelpNY (https://homeownerhelpny.org/), an online service that provides New Yorkers with the tools and support to avoid foreclosure and/or scams, and to help them find qualified mortgage assistance relief services from the HOPP network. HomeownerHelpNY is a collaboration between the New York State Attorney General Office and the Center for NYC Neighborhoods, a non-profit committed to protecting and preserving affordable homeownership.
