In coordination with Saint Francis Hospital, the Saint Francis Hospital Outreach Bus provides free transportation for services: Nursing Services include blood pressure screening and monitoring of other chronic conditions, health information and/or education, in-home assessment, medication management counseling and information, referral to home healthcare services, healthcare coordination, fall prevention screenings and education, and brain games. Social Work Services include individual, family and group counseling, In-home assessment, case management, bereavement support, caregiver assistance, advance directive education, entitlements/Benefits assessments and assistance (food stamps, home heating, low-income programs), disability resource information, financial counseling (referrals), food and nutrition support (Meals on Wheels, Food Pantries, referrals)Medicare, Medicaid or other health insurance information, mental health referrals, respite care (referrals), transition assistance from hospital to home or rehabilitation, home maintenance referrals, support groups, Veterans assistance, Circle of Support (Life planning tool).

The Mid-Hudson Library System is a state cooperative agency which brings together the resources of public libraries located in Columbia, Dutchess, Greene, Putnam and Ulster Counties. MHLS is dedicated to making local libraries better while saving local tax dollars. Since 1959 MHLS has worked with its member libraries to insure the public's right to free access, economical resource sharing and professional library services. Family Defense Unit which helps parents navigate the challenges of the Child Welfare system. Help with CPS investigation against parents; clearing parents name at the State Central Register for child abuse and neglect; neglect and abuse court cases; permanency planning court cases and termination of parental rights.

Provides companionship and non-medical support services to those struggling with the challenges of aging and/or medical impairment. Support services include meal preparation, light housekeeping, transportation to activities and appointments, shopping, errands, escort for walks and family gatherings and attention to well being. A consumer driven personal assistance program that allows people with disabilities to gain more control over their personal assistance services. CDPAP enables self directing consumers to advertise, recruit, interview, hire, supervise, train and dismiss their own personal assistant(s). Self directing persons assess their own needs, determine how and by whom these needs should be met, and monitor the quality of services received. Individuals independently make all decisions and manage services directly. If an individual is unable to do so, they may use a certain family member or friend to manage needed services. Living Together is a forum for discussion of unmet service needs using advocacy, community education, outreach, lobbying efforts and collaboration with service providers to improve access to care and support those needing services.

Offers case management for grandparents, aunts, uncles, or close family friends who have custody or informal care referrals for legal services and other supportive services, caregiver and youth support group and education, school advocacy, family and recreational activities, food, clothing, and assistance with other needs, in addition, Holiday gifts.

Provides a licensed residential treatment facility for adolescents with severe psychiatric conditions. Services includes an open setting with intensive therapy for the child, with special education, pharmacotherapy, and vocational training. Group and family psychotherapy is also part of the program.

A social and recreational program for youth-at-risk. School aged children and adolescents participate in programs including community service projects. The Center plays an important role in developing positive peer relationships and building a youngster’s sense of self-esteem and worth. A safe and supportive after school program is created by staff in order to enhance the safe socialization and educational process of students, as well as decrease the students’ likelihood of involvement in at-risk behaviors, such substance use, gang involvement or violence. Activities implemented to facilitate this process includes physical exercise, homework assistance, therapeutic arts and crafts, and a drop in social and recreational program. When necessary, participation in a mentorship program and/or participation in evidence based intervention/prevention programs such as Teen Intervene or Too Good for Drugs & Violence, or on-going preventive therapeutic counseling for the student and family.

Provides legal representation and family support services to income eligible families involved in a first time Child Protective Services(CPS) investigation prior to the petition being filed in Family Court. Support services may include social workers, interpreters, investigators, immigration attorneys and eligibility screening staff. Provides Information and Referral, Reference Services, Readers Advisory and programs for children from birth through grade 8 as well as parents. Programs and services include, but are not limited to, topics such as parenting workshops, storytimes, reading clubs and arts and crafts.

Provides a 24 hour Emergency Communications system to keep military personnel in touch with families during family emergencies, such as illness, death, births and provides military personnel and their commanders the information needed to enable emergency leave, compassionate reassignment, and dependency discharge. Secures emergency financial assistance. Provides counseling, referrals, and other social services. Provides support services to patients in Veterans Administration Hospitals.

Provides diagnostic and mental health evaluations for adolescents and young adults who have experienced school failure due to biological-based difficulties, learning disabilities, limited intelligence or attention problems. Treatment usually includes individual or group psychotherapy, family counseling, psychoeducational intervention, and speech/language services. Treatment can be on-site or referrals are made to community treatment centers.
