Offers Abortion Care, Birth Control, Breast Exams, Colposcopy, Emergency Contraception, GYN Care, Gender Affirming Hormone Care, Health Counseling, HIV testing, Counseling and Education, HIV Prevention (PrEP), HPV Vaccine, Pap Tests, Pregnancy Testing and Options Counseling, Smoking Cessation, STD testing and treatment. Planned Parenthood health centers support and welcome patients of all gender identities and sexual orientations, and offer care regardless of citizenship status.

Residential and Shelter Program: Open to all victims of domestic violence and their children. Provides information and referral, advocacy, crisis intervention, counseling, support group, and case management. Non-Residential Services: Chautauqua County Domestic Violence Rape Crisis Hotline, information and referral, advocacy, support groups, crisis intervention, and case management. Children and Family Outreach: Provides group and individual counseling services to children who are past or present victims of domestic violence, sexual abuse, and/or child abuse. Education and Outreach: Offers presentations in schools and the community on a variety of topics, with a focus on domestic violence and sexual assault. Support Groups: For women and children; call hotline for meeting times, location, etc.

Assists families with stabilizing youth in their home, school, and community through linkage to community-based services. Responsible for Children Services Intake. Offers a component for PINS (Persons In Need of Supervision) Diversion and assists with runaways. Other services include information and referral, assessment and case planning, and preventative services. Program is strictly voluntary.

Provides information, referral, and case assistance services to persons 60 years and older living in Niagara County. Assistance can also be provided to family and friends caring for an older person.

A committee designed to link families who may require more intensive mental health support to services such as Care Coordination, Home-Based Crisis Intervention, Mental Health Respite Program, Home and Community Based Services, Family Support, and OMH services such as Residential Treatment Facility, Community Residence, Children's Psychiatric Center, or mobile crisis programs. Families are encouraged to invite schools and others working with their child to attend a meeting; approach is strength-based and constructive; jointly crafted written plans are provided to families at the conclusion of the meeting. Committee is made up of representatives from the following organizations: Wyoming County Department of Mental Health, Social Services, Probation, and Youth Bureau; Spectrum Health & Human Services (Certified Community Behavioral Health Clinic Services including Mental Health Counseling and Chemical Dependency Services), Hillside Children's Center, Wyoming County Community Hospital (Crisis Outreach), GLOW Family Support, Clarity Wellness Community, New Directions and Western New York Children's Psychiatric Center.

Provides nursing, social services, home health aides, social workers, infusion therapy, home health evaluations and assessments, nutritional counseling, arranging of medical equipment and supplies, personal care/homemaker services, adult day care, and physical, occupational, and speech therapy. Long term home health care program available. Specialized programs available in pediatrics, behavioral health, and wound care. In-facility assessments for discharge planning at hospitals and skilled nursing facilities available upon request. Serves as a state provider in the MOMS (Medicaid Obstetrical & Maternal Services) Program, providing comprehensive support nursing services to prenatal and postpartum women.

Provides health care services including:

• Adult & pediatric primary care, physical exams, immunizations
• HIV/AIDS, hepatitis C, and STI prevention, testing, and treatment
• Specialty in LGBTQ+ and transgender care and care for individuals with intellectual and developmental disabilities
• Chronic disease management services (HIV/AIDS, diabetes, asthma, obesity, etc.)

Democratically run supportive living for persons in recovery from drug and/or alcohol addiction. Use of drugs or alcohol, disruptive behavior, and non-payment of rent are causes for dismissal from the house.

Provides crisis intervention and support services to anyone who is a survivor of sexual assault or domestic violence and to their families. All services are confidential. Trained counselors are available 24 hours a day, 7 days a week to provide information and support, enabling clients to make informed choices concerning medical, legal, and counseling needs, while offering advocacy and information about their rights. Counselors offer short-term counseling (6-8 sessions); medical, legal, and court accompaniment; crisis intervention for sexual assault survivors and their significant others; support through medical examinations and treatment; advocacy and support during interaction with law enforcement; individual consultations with professionals; community education programs; professional training programs; and referrals for additional needed services. Serves as the point of contact for domestic violence shelter placement in Wyoming County. A bi-lingual, bi-cultural outpatient substance abuse treatment program. Services include individual and group counseling, medication assisted recovery (including methadone, buprenorphine, and naltrexone), personalized health assessment, integrated care management, relapse prevention, benefits/entitlements assistance, educational/vocational placement, and linkage and referral to other needed services. ##REENTRY##

Home visiting nursing program that provides prenatal and postnatal supportive services. MOMS is designed to improve equitable access to quality maternal child health care, increase lactation support for parents choosing to breast feed, and enhance the coordination of care with other community resources. Program services include: Health Screening & Monitoring, Pregnancy Education, Nutrition Screening & Monitoring, Infant Care Education, Lactation Counseling, Postpartum Mental Health Education, Childbirth and Family Planning, Early Childhood Development, Community Resource Referrals, and help with WIC program enrollment (if appropriate) and making an appointment with a participating OBGYN. Staff RN is also a Certified Lactation Consultant (CLC) who can answer questions about yourself and your newborn in regard to breast/chest feeding. Investigates child abuse and maltreatment reports to protect children (under 18 years old) from further abuse or maltreatment and to provide rehabilitative services to children, parents, and other family members involved.
