An alternative program that serves disabled youngsters of high school age who present a variety of educational and emotional needs. A failure to succeed in school is often tied to a variety of emotional problems frequently linked to a disturbance within the family unit. This program provides an enriched educational environment that incorporates the Common Core Learning Standards. They receive ongoing services from consulting psychiatrists, either employed by the Office of Mental Health or BOCES. Transitional classes are available for those students capable of being mainstreamed into a regular high school. An after-school program is offered for students who are unable to attend a normal full-day school due to the students’ disability(ies). This option is also suitable for students who are employed during the school day and who require after-school educational programming to complete their high school studies. Related services, support services and psychiatric consultation services are also offered for students who require these specialized services. Also offered is the PASS Program at Brennan High School. This is a highly structured environment in which instructional and behavioral deficits can be addressed within a small group setting for high school aged students. Students may or may not be classified as special education students. All are in need of alternative education. Truancy, non-compliance with school rules and generally disruptive behavior often characterize a student in crisis; such a student is appropriate for the PASS Program. Students who are at risk due to poor achievement may also be referred to this program where they will be afforded many support services to ensure success.

CARE MANAGEMENT | HCR HOME CARE - PLATTSBURGH OFFICE
Care Management services for Adults and Children who have physical and/or mental health diagnosis. Care Managers will assist with scheduling appointments, arranging for transportation and will attend appointments at the clients request to help advocate for the clients wants and needs. A Care Manager for a child will work with the parents or guardian to ensure a child is safe, healthy and well cared for. We will work with providers including the schools to ensure a child's voice is heard and goals are developed to meet the child's needs.

Psychiatric day treatment program for youth in grades 6-12 that may benefit from enhanced clinical care within an academic setting. Provides educational and clinical services to students in a 6:1:1 classroom setting at a standalone facility. Students receive educational services along with individual therapy, group therapy, family therapy, and medication management when necessary. Students can also access crisis support services when therapeutically necessary.
Integrated mental health services are provided within a strength-based recovery model. Partners with Erie 1 BOCES component school districts, Buffalo Public Schools, and families in the referral process, and collaborates with both families and school districts to reintegrate youth into a less restrictive academic setting. Also assists with linkages to community support services to support recovery. Average length of outpatient treatment is 3-6 months or as soon as the student meets their established treatment goals to be discharged to a less restrictive setting.

Works with individuals living with mental illness who have been non-compliant with past treatment which has led to multiple hospitalizations, threats, or acts of violence toward self or others. If an individual is not able to be engaged in appropriate services, a court petition may be filed to request an order for AOT. Recipients are monitored for treatment compliance and to ensure provision of appropriate services.
