Care Managers help identify and link clients and their families to services that can help meet their medical, social/emotional, behavioral, and basic needs. Clients are youth up to 21 years of age with active Medicaid, diagnosed with chronic physical and/or mental health conditions, HIV/AIDS, or Complex Trauma.

Supportive community living for parenting youth ages 14-21 in the foster care system. Provides the opportunity for youth to raise their children in a supportive home while working on goals related to permanency, self-sufficiency, and parenting. Each resident contributes to the care of the home by doing daily chores and preparing meals. Youth participate in regular community meetings related to issues within the home. Youth are connected to school, work, vocational training, day care services, medical services, and mental health services. Facility is comprised of 8 bedrooms, a shared dining room, living room, kitchen, and bathrooms.

A multi-disciplinary program responding to reports of physical and sexual child abuse. Criminal justice, medical, and mental health services are coordinated on site. Prosecutors, police officers, social services workers, therapists, victim advocates, and medical professionals work together to investigate allegations and to reduce trauma for children reported as victims of sexual or physical abuse. Provides a child-friendly environment for interviews and medical examinations. Also provides community education and professional training on issues related to child abuse and family violence.

Care coordinators assist clients with:

• Finding a doctor or specialist
• Making sure that doctors are talking to each other and the client about their care
• Advocating for the client
• Helping the client understand and remember confusing information
• Scheduling appointments, tests, and more
• Getting care when the client is discharged from the hospital
• Getting into mental health counseling or substance use programs
• Finding transportation to medical appointments

Group for adults with a mental health diagnosis interested in using writing in their mental health recovery. Meets in person on the 1st and 3rd Saturdays of every month at 1021 Broadway on the 4th floor from 10am-12pm. Also meets virtually on Monday's at 1pm. A 24-hour state-operated supervised community residence with 24 beds. Offers support and skills training to individuals preparing for more independent living environments.

Provides outpatient mental health services to clients who seek help for depression, anxiety, suicidal feelings, and psychiatric illness.

Substance abuse prevention program for Wyoming County. Professional prevention specialists deliver evidence-based prevention programs to Wyoming County school districts and communities. Programs include responsible server training, underage gambling awareness, alcohol, tobacco, and drugs training, substance abuse prevention skills training, teen intervention, and more. Encourages partnerships between traditional and nontraditional groups in the community such as youth, schools, probation, businesses, libraries, media, youth-serving agencies, and faith-based organizations, parents, and other concerned adults. Through such partnerships, this community-based coalition provides prevention strategies that target under-age drinking, cigarette smoking, vaping, marijuana use, prescription drug abuse, and use of heroin and other drugs.

This program is a peer to peer, non-crisis telephone service that provides support and encouragement.

Provides outpatient mental health services including individual, marital, family, parent, and group therapy; depression and anxiety counseling; psychiatric assessment and treatment; medication management and provision; crisis intervention and case management; specialized programs in stress management; domestic violence intervention for victims and perpetrators; and trauma recovery.

Offers comprehensive services to children and families, which promotes literacy skills and social, mental, emotional, and physical needs. Parents are the cornerstone of the Head Start Program. The Head Start staff recognizes that parents are the child's first teachers. We encourage parent involvement in all aspects of programming from classroom volunteers to serving on decision-making committees. Provides safe settings, Kindergarten readiness skills, classroom and home-based experience, hot nutritious meals, health screening, community referrals, transportation, non-violent problem solving skills and day care referrals for children and siblings.

Provides comprehensive treatment, recovery, and support services for individuals and families impacted by mental health and substance use challenges. Services offered include individual and group counseling, medical and psychiatric care, family support coordination, addiction medicine consultations, opiate overdose prevention training, recovery coaching and peer support services, care coordination, community outreach and education, harm reduction services, a therapy dog program, and more. Outpatient program including alcohol and drug counseling on a regular basis. Length of program is 6 months. Accepts former offenders with mental health issues. Works with New York State Parole. ##REENTRY##

The Center offers multi-disciplinary assessments and evaluations for children suspected of having a developmental disability, including autism spectrum disorder. If the results of the assessment indicate the need for treatment, the team will provide individualized behavioral programming, occupational therapy, speech therapy and medical management including developmental pharmacology. Also specializes in the treatment of children, adolescents, and adults with behavioral, emotional, and psychiatric difficulties.
