Whitney M. Young, Jr. Health Services provides primary medical services to the uninsured and underinsured residents of Albany, Columbia, Fulton, Greene, Hamilton, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, and Washington Counties.(see eligibility) Programs include prenatal care, women's health, pediatrics, adult medicine, family practice, school based health, mental health, nutrition, eye care, and podiatry services. The Troy office provides family practice and women's health services.

Provides two different group living environments: Supervised IRAs: Offers small group living environments with 24 hour staff supports for a small number of adults with different abilities and are encouraged to be as independent as possible. Individuals received supports and services based on their individual needs. Supportive IRAs: Offers supportive apartments where 1 to 2 people live in the same apartment and receive supports from staff. Individuals must be able to remain at home without staff, evacuate in case of an emergency, call for help and interact safely with neighbors, if needed.

Responds to individuals in crisis, over the phone or in person, in order to reduce emergency room visits. Provides mental health engagement, intervention, and follow-up support.

Provides supportive housing for women with a diagnosed mental illness, substance abuse, or dual diagnosis. Services include case management, preventative services, life skills training, medical and mental health services, vocational and job training, education and employment opportunities.

Provides mental health counseling and medication management for individuals ages 5 years and older.

Catholic Charities acts as a Representative Payee for managing Social Security or SSI benefits for individuals who are unable to do so themselves. Payees ensure benefits are used for current needs like housing, food, & medical care, keeping accurate financial records and reporting any changes to Social Security Administration. Any remaining funds that are not used during the month are kept for the individual's future needs. Community referrals are made through the Department of Social Services.

Provides individualized peer-to-peer outreach, advocacy services that protect the rights and interests of the individual, supportive self-help groups and activities to promote wellness, and acts as a clearinghouse of information related to self help, mutual support, rights, natural community supports, and alternatives.

Provides psychiatric outpatient services to clients ages 18 and older.

NYS OMH licensed residential program that provides 24 hour supervised congregate setting for 8 adolescent boys with a primary mental health diagnosis. Provides case management, support in daily living and the development of independent living skills, counseling, recreation, medication management and understanding symptoms, family support and relationship building.

Provides a comprehensive pre-school program for economically disadvantaged children, or children with disabilities, ages 3 to 5 years. Services include education, health, mental health, nutrition, parent involvement, and social services.

Offers clinical services including mental health counseling, family therapy, parent education, and social skills building for students and families of Whitney Point, Harpursville, Windsor, Johnson City, Chenango Forks Union Endicott Central Schools and Binghamton East Middle.

Provides recreational and creative arts activities in a highly supervised after-school setting for youth ages 7 to 12 years with mental health needs. Offers a range of small group activities, such as gym, recreation, music, and arts and crafts as well as homework assistance. Provides transportation to and from the program.

• Provides Assistance for Special, Cognitive, Emotional Needs & Disabilities. Program will allow residents with preexisting mental health concerns, developmental disabilities, health conditions, or religious or cultural considerations to voluntarily submit their information to the county so that law enforcement and fire and EMS personnel are aware of their needs and can provide the proper support and resources. Information volunteered to the ASCEND program is strictly confidential. some of the condition that can be reported are:

  • Trauma ans/or Stress Disorder,.
  • Neurocognitive Disorder.
  • Religious or Cultural Considerations.

  * Offers the option to provide first responders with any potential triggers, calming methods, communication impairments and frequented locations

Provides case management services to adults with chronic health conditions, serious mental illness, or HIV/AIDS to assist them in staying healthy, out of inpatient care, hospitals, and emergency rooms.
