Provides a PINS diversion program. AFY is a 30-45 day home based crisis intervention for youth that display a pattern of incorrigible behavior(Defiance in the home, disobeying curfew, aggressive behaviors, truancy/failing school and mental health issues). Case Managers will do an assessment of family needs and links them with community resources to get the family out of crisis. In addition, after the 30-45 days are completed. a Phase 2 of the program is implemented to link the families with partnership agencies for typically 3-6 months, but can be up to a year to ensure the ensure the family's needs are met and they remain stabilized. AFY also provides parent support workshops, adolescent male and female workshops as well as peer mentoring services.

Provides services to avoid placement of children and youth outside their homes. Family- focused case management services. Helps to keep families intact and to avoid problems that may cause children to be at risk. PERSONS IN NEED OF SUPERVISION (PINS) in cooperation with the Dept. of Probation, provides intensive case management services to youth placed into the Persons in Need of Supervision (PINS) Diversion Program. Support services are offered to families.*PINS* parent orientation & information session conducted monthly for parents/guardians interested in finding out more about program. YAP continually evolves services to meet new needs that emerge for youth involved within the juvenile justice system, but most of our programming is designed for the purposes below. Diversion: to divert youth from formal system involvement. Pre-Adjudication: to prevent formal detention while ensuring community safety. Post-Adjudication: to prevent incarceration in prisons, jails or other residential or secure facilities. Re-Entry: to promote safety and support to youth transitioning back to the community from institutional placement.
A 6-month program where convicted individuals have an opportunity to get help without going to jail or prison. This program must be completed in order for the individual to be set free of jail/prison time by doing a variety of services, such as community service, schooling, job training, alcohol/drug treatment, if applicable and intense counseling.
A program where convicted drug users visit the court once a week and are involved in an inpatient or outpatient drug counseling program four days a week under the guidance of a treatment coordinator, which also involves drug testing very often.
Provides in house self-help groups at Suffolk County Jails in order to assist clients in their transition from jail to the community. Clients served are those who are over 18 and who have a mental health condition that was in evidence prior to incarceration.
