Provides medication for Alzheimer's Disease and other cognitive disorders group and/or individual therapy medication for psychiatric and behavioral disturbances psychological treatment approaches caregiver psychoeducation and support referral to other relevant community resources opportunities to participate in ongoing research on medications to improve memory. Case managers provide a comprehensive in-home assessment to determine the client's needs and link them with the appropriate community services, programs and entitlements that help them age safely in their homes. New York State and Nassau County funding may provide up to 20 hours of aide assistance weekly. Chore workers can provide a few hours of light housekeeping and shopping for those who do not require hands on care.

In coordination with Saint Francis Hospital, the Saint Francis Hospital Outreach Bus provides free transportation for services: Nursing Services include blood pressure screening and monitoring of other chronic conditions, health information and/or education, in-home assessment, medication management counseling and information, referral to home healthcare services, healthcare coordination, fall prevention screenings and education, and brain games. Social Work Services include individual, family and group counseling, In-home assessment, case management, bereavement support, caregiver assistance, advance directive education, entitlements/Benefits assessments and assistance (food stamps, home heating, low-income programs), disability resource information, financial counseling (referrals), food and nutrition support (Meals on Wheels, Food Pantries, referrals)Medicare, Medicaid or other health insurance information, mental health referrals, respite care (referrals), transition assistance from hospital to home or rehabilitation, home maintenance referrals, support groups, Veterans assistance, Circle of Support (Life planning tool).

Provides specialized medical care in all major fields of medicine.

Offers elderly or severely disabled individuals residing at home a day program providing nursing and medical care, case/care management and recreation. Services include access to a variety of medical specialists (dentist, podiatrist, audiologist, therapists), and individualized nursing care for a wide range of medical conditions associated with chronic illness and aging including Alzheimer's, dementia, diabetes and Parkinson's Disease. Program attendance is based on individual needs. Transportation to and from the Center is available.

Case management for seniors with behavioral health needs. A team including an RN, case counselor and peer will provide confidential, comprehensive in-home assessment, develop a care plan and provide intensive short-term case management for seniors 55+ struggling with behavioral health, substance use or cognitive impairments that interfere with accessing community services and resources. Barriers to accessing services will be addressed.

GMT Team has the capacity to travel and visit clients in their own homes, adult care or nursing home for the purpose of making assessments, monitoring the client's response to treatments and providing emotional support to the clients and/or family members.
