Treatment provided as indicated by examination results for genital warts, genital herpes, and other conditions such as crabs and scabies. Patients qualifying as high-risk are also screened for Hepatitis C. Confidential rapid HIV testing available at the request of the patient. Post-test counseling and referrals available for all patients tested for HIV. Also provides referrals for PrEP (Pre-exposure Prophylaxis) for HIV. Nurses provide counseling and education on PrEP to all patients servicing the Sexual Health Center. Individuals are screened to see if PrEP is the right choice for them. If an individual is a good candidate for PrEP, one of the clinic nurses will work with them to make an appointment at Niagara Falls Memorial Medical Center Infectious Disease Clinic. Provides free condoms, finger cots, lubricant, and dental dams. HOURS: Sexual Health Center Clinic: Tuesday & Thursday, 9am-3pm. ProAct County Discount Card Program. There is absolutely no charge to you or the county to belong to this program. This card was designed to be used if you do not have insurance or if something is not covered by your current insurance program. This card cannot be used in conjunction with another insurance program in order to discount your co-pays or deductible. Benefits include 10-20% off on brand name drugs, 20-70% off generics. Enrollment: FREE for Your County and FREE for participants. Every prescription medication, including some pet medications, is eligible for a discount. A season program each year, people on Medicare can enroll in the Medicare prescription drug program, to help save money on prescriptions. People enrolled in both Medicare and Medicaid are automatically signed up for the Medicare prescription drug program.

Provides home care services for those who may need continued care due to age, illness or disability. Services include home nursing, home health aides, personal care, homemaker services, physical, occupational and speech therapy. Tele-health is also available which allows an individual to be monitored by a health professional from the privacy of home. Also offers personal emergency response systems that puts an individual in touch with emergency assistance at the press of a button. Also available is Med-Ready:a medication dispensing unit which helps a patient to take the proper medication at the proper time. This helps to avoid over dosages and medication interactions.

Federally Qualified Health Center (FQHC) offering pediatrics, internal medicine/family medicine, OB/GYN, pediatric and adult dental services, podiatry, social work, nutritional counseling, medications for opioid use disorder, and pharmacy with home delivery. Serves all populations with special focus on the medically under-insured, uninsured, and those experiencing homelessness. Also provides onsite lab, ultrasound, STD testing, COVID-19 testing and vaccines, health education/groups, blood pressure screening, and free walk-in pregnancy testing. Provides transportation assistance to help clients get to appointments and referral assistance.
