Offers five Family Health Centers which provide essential primary care services and “medical homes” to residents of some of Long Island’s most underserved communities. By bringing quality primary care and a growing number of specialty services out into the community, we’re helping more Long Islanders get the care they need, where they need it most. Provides comprehensive primary health care services, such as preventive care, general physical exams, immunizations, cancer screenings, lab for medical testing, including pediatrics, family practice, minor surgery, and women's health.

Offers a full-range of non-emergency primary care services for those who cannot get in to see their primary care provider. Services include treating injury, illnesses, routine check-ups.

* Provides specialized clinical and interpreter health care programs for individuals from Southeast Asia, Russia, Ukraine, Somalia, and Vietnam. Services available at all Anthony Jordan Health Center locations. However, only Brown Square Health Center currently offers live interpreter services.

* Provides Public health services related to: Chronic Diseases Communicable Diseases Emergency PreparednessRabies Prevention Immunizations Lead Poisoning Prevention Sexually Transmitted Diseases Tuberculosis Maternal Child Health, including breastfeeding support, childbirth classes, and newborn care classes Early Intervention and Special Education Preschool Stop DWI program

Preventive health and wellness services, including cancer screening and promotion of a healthy lifestyle, Laboratory services, Endocrinology services, and patient education materials.

Provides a variety of medical services including route medical care, physical examinations, patient education, smoking cessation, immunizations, STD and HIV diagnosis and treatment, and contraception management.

* Offers general medicine services including diagnosis and treatment for general medical problems, chronic disease control, and school and work physicals. * Offers Addiction Medicine, including prevention, screening, intervention and treatment for substance abuse addiction, with a focus on opioid and alcohol use.
