Epidemiology & Disease Surveillance - ECDOH

Primary services include: - Monitoring the community for the presence of communicable diseases; - Providing information on communicable diseases, including preventative measures, to members of the medical community, the public, and individuals exposed to communicable diseases; - Investigating outbreaks or unusual occurrences of disease, then instituting or recommending appropriate control measures; - Investigating foodborne illnesses that occur in Erie County; - Coordinating follow-up on animal bites and providing information on the treatment and risk of rabies; - Compiling a community health assessment that provides information on births, deaths, etc., as a resource for planning or evaluation.

Healthy Living with Diabetes - UMMC

Provides a caring community that understands the challenges of living with diabetes. Helps individuals to obtain knowledge, skills, and support to better manage prediabetes and diabetes. Programs include: - Diabetes Prevention Program: For those with prediabetes or at high risk for type 2 diabetes. A year long program focused on implementing lifestyle changes to reduce the risk of developing type 2 diabetes. - Diabetes Counseling: For individuals with prediabetes or diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support. - Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications. - Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.

Health Education - Orleans County Health Department

Develops the Community Health Assessment / Community Health Improvement Plan. Promotes health, safety, and healthy lifestyles. Services are provided through free resources and referrals, public presentations and programs, speakers, in-service training seminars, workshops (including T2 Diabetes Prevention workshops), social media, and free literature on numerous health concerns geared towards all ages and literacy levels. Prediabetes/Diabetes Classes - Orleans Community Health/Medina Memorial Hospital

Offers classes with an evidence based curriculum (PreventT2) proven to prevent or delay the progression of prediabetes into the onset of type 2 diabetes and other serious health problems by introducing realistic lifestyle changes and goals. Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Approximately 84 million American adults have prediabetes. Of those with prediabetes, 90% don't know they have it. Diabetes Management Programs: Help people with diabetes to take control of their diabetes and live healthier lives. An expert diabetes team (Certified Diabetes Educator/RN, Registered Dietician, Health Educator, Pharmacist, etc.) teaches people what they need to know to feel better and help avoid complications of diabetes. Classes, support groups, and one-one education available.
