  • Diabetes Prevention Program: For those with prediabetes or at high risk for type 2 diabetes. A year long program focused on implementing lifestyle changes to reduce the risk of developing type 2 diabetes.
  • Diabetes Counseling: For individuals with prediabetes or diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support.
  • Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications.
  • Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.

Offers classes with an evidence based curriculum (PreventT2) proven to prevent or delay the progression of prediabetes into the onset of type 2 diabetes and other serious health problems by introducing realistic lifestyle changes and goals. Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Approximately 84 million American adults have prediabetes. Of those with prediabetes, 90% don't know they have it. Diabetes Management Programs: Help people with diabetes to take control of their diabetes and live healthier lives.
