MHA provides funding stream for individuals with developmental disabilities, that creates more independent housing opportunities, such as those who live independently and require additional supports, or for those leaving home, a community or institutional facility, or those who are homeless.
Case managers provide a comprehensive in-home assessment to determine the client's needs and link them with the appropriate community services, programs and entitlements that help them age safely in their homes. New York State and Nassau County funding may provide up to 20 hours of aide assistance weekly. Chore workers can provide a few hours of light housekeeping and shopping for those who do not require hands on care.
