Assists in applying for New York State sponsored health insurance through Medicaid, as well as Managed Long Term Care (MLTC) and Nursing Home Coverage. Facilitated enrollers help go over paperwork and put together all of the information needed to apply. Helps apply for Medicaid as a supplement to existing coverage such as commercial insurance, Medicare, or VA Healthcare. If eligible, Medicaid can pay for remaining costs that insurance doesn't cover, including co-pays, dental, and prescriptions.

Medicaid provides medical coverage for individuals based on income and resource guidelines.

Family Assistance: Provides temporary financial support to families with children and pregnant women who are without sufficient income to maintain themselves. Safety Net: Provides vouchers for essential expenses to individuals who have reached their lifetime limit of Family Assistance and children in the care of non-relatives, or for single and childless couples. Temporary Disability Assistance (TDA): Provides assistance to persons with temporary disabilities, persons 60 years of age and older, caregivers of those with disabilities, and those with permanent disabilities awaiting SSI benefits. SNAP (Supplemental Nutrition Assistance Program): Provides assistance purchasing food for individuals and families with limited income. Mixed Household SNAP benefits are granted to households who have some members who receive temporary assistance and some who do not. Benefits are obtained electronically through the EBT system. Medical Assistance: Provides medical coverage to persons without insurance, premium support to persons paying a Medicare premium and coverage for adults whose children receive Child Health Plus. Rental Assistance: Provides assistance with rental payments to eligible households, as determined by the application process.

Provides screening for benefits programs including Food Stamps/SNAP, HEAP, Medicare Savings Programs, and Medicaid. As appropriate, provides referrals to agencies such as the Social Security Administration, Department of Social Services, and Veterans Service Agency.

Accepts applications for temporary assistance, Medicaid, SNAP, and emergency assistance. Homeless team provides emergency shelter placements for eligible clients.

Provides Personal Care Aides and Home Health Aides 24 hours a day, 7 days a week. Trains personal care aides and home health aides. Offers a Traumatic Brain Injury (TBI) program; Home and Community Support Services (HCSS) Aides provide daily living supports to TBI program participants. The goal of the TBI program is to meet the needs of individuals with traumatic brain injury who require nursing home level care, but who prefer to live in their homes or private residences.
