Provides essential medical services for persons receiving Public Assistance and other low-income persons. Services covered include inpatient and outpatient hospital care, physician services, skilled nursing care, laboratory and x-ray services, early and periodic screening of children, home health care services and transportation services. Call for detailed information regarding services offered. Medicaid can cover incontinence supplies, including diapers, for children and adults who meet specific requirements.

Provides the connection for low income families with children under 19 to access health insurance for their children.

Individuals with both Medicare and Medicaid, who are 21 years of age or older, seeking information on various Medicaid home care programs should call New York Medicaid Choice at 1 (888) 401-MLTC (6582). Other individuals not enrolled in Medicaid Managed Care seeking information regarding non-institutional care such as personal care aide services, should call the Home Care Intake Unit at (631) 854-9585. The Independent Consumer Advocacy Network (ICAN) is an independent, conflict-free entity providing Individuals free assistance in accessing care, filing grievances, appealing adverse decisions, and understanding and exercising rights and responsibilities. There are 60 physical locations throughout NYS for in-person assistance, a call center (1-844-614-8800) for the entire state, and a website icannys.org. ICAN currently serves MLTC plan enrollees and mainstream Medicaid managed care enrollees who receive long term care. Call for detailed information.

Information centers which provide seniors with the latest in Health & Medicare Insurance Medicare D and a computer analysis of drug providers based on current medications. Lists are printed of drug providers, contact information and neighborhood pharmacies. Social Security Extra Help applications will also be available.

Affords Medicaid-insured Consumers (individuals being cared for) the opportunity to live in the community as independently as possible while taking on the responsibility of an active, consumer-directed role in the planning and implementation of their own homecare.The CDPAP is a long-term homecare program that enables the service recipients to receive Personal Assistance Services free of the constraints that would otherwise be imposed upon them by a more formal health care environment.The CDPAP can enable a Consumer to hire & direct their Personal Assistant (PAs) in the provision of a wide range of services based on the Consumer's approved care plan that can include basic housekeeping, personal care or high level nursing services.

Social Services promotes self-sufficiency by providing financial and social services to eligible County residents.Able-Bodied Adults Without Dependents (ABAWDs) are SNAP recipients aged 18-64 who are able to work and don't have a dependent child in their household. They can only receive SNAP benefits for a limited time (3 months in a 36-month period) unless they meet certain work requirements or have an exemption. These work requirements generally involve working or participating in a work program for at least 20 hours per week (80 hours per month). NY Connects is able to assist clients with EPIC and Medicaid application process.
