Offers Consumer Directed Personal Assistant Services (CDPAS) that allows individuals with disabilities and/or chronic health conditions to hire, monitor, train, and terminate personal care attendants of their choosing.

Provides vocational assessment, job readiness training and job placement. Refers individuals to appropriate government programs and services. The level of care provided depends on the need, which is outlined below: Home Health Aide (HHA) - Provides assistance with feeding, dressing and bathing. HHA's monitor temperature, pulse rate, respiration rate, and blood pressure. Personal Care Aide (PCA) - Provides health care tasks, activities of daily living including personal hygiene services, housekeeping tasks essential to the patient's health and other related supportive services. Companion/Homemaker - A program that provides assistance with house cleaning, laundry, meal preparation, reading, conversation, grocery shopping, prescription pick-ups and errands. Private Pay Care - Private Pay Care encompasses any of the above services. Contact office for private pay rates if not contracted with your insurance company. Skilled Nursing - Provides skilled nursing services such as medication management, diabetic management, wound changes, catheter care, injections. Vary depending on agency availability.

CDR serves as the "employer of record" and maintains responsibility for maintaining personnel and payroll records as well as billing Medicaid and assists participants with recruitment and problem solving.

Allows individuals with physical disabilities or chronic illnesses to choose, hire, train, schedule, supervise and dismiss their own personal assistants. Personal assistants can be a relative, friend or neighbor. No certifications are needed. Care may include any of the services typically provided by a personal care aide, home health aide or private duty nurse, such as housekeeping, cooking, bathing, dressing or giving medications.
