* Offers people with intellectual and developmental disabilities and their families assistance to navigate services and supports. Promotes informed choices and independent decision-making. Care Coordinators work continuously to connect people to the care, support, and opportunities. Monitors circumstances and services during life transitions.Coordinates access to medical, dental, and behavioral services.Keeps track of safety and residential needs.Referrals to community and social support services.Creates a holistic Life Plan and adapts the plan as new goals and needs arise.

geriatric mental health: counseling, medication (evaluation, consultation and management) and referrals for elders and their families. caregiver counseling: ongoing education, supportive counseling, communicaton strategies and mental health care options through monthly meetings with a licensed clinicial social worker for support and obtain education. caregiver support group: free, therapist-facilitated group on caregiving issues. Participants need not be a Family & Children's Services client.

* Provides ongoing programs for LGBTQ+ and ally caregivers and older adults including: Care management. Personal safety advocacy and support. Support groups. Coffee hour for caregivers.

* Provides community mental health clinic services for adults and children. Services provided include: Individual mental health therapy for children (starting at age 4), adolescents, and adults. Group therapy. Medication management. Nursing and care management services.

* Coordinate healthcare, personal services, social supports, disease-related care for chronic conditions, and access to preventative care. * Provides service planning and assistance in securing access to services to help individuals in their recovery.* Assists adults, young adults, and youth that are experiencing chronic medical, mental health and/or substance use disorders conditions with care coordination and related community-based services.

* Helps Veterans with Post-Traumatic Stress Disorder, Traumatic Brain Injury, and Military sexual trauma regain their lives by providing quality clinical and case management. Services include: Individual mental health counseling. Group therapy. Support therapies (Art). Case management. Specialized groups to support reintegration.

* Offers a broad array of services which include: Treatment Residential: Community residences. Supportive apartment programs. Single room occupancy (SRO). Crisis Respite. Supported housing. Transitional housing. Children and Youth Care management. Adult Health home care management. Forensic case management. Outreach and non-medicaid case management. Recovery support services for individuals recovering from both mental health and substance use disorders.

In collaboration with New York State Community Service Society, assist people who are enrolled in New York State Medicaid manage care plans and those who receive long-term care services; such as questions regarding Medicare and Medicaid programs for those receiving long-term care, managed care option, help to resolve problems with plan and providers, help in lodging complaints or appeal health care decisions. Medicaided Facilitated Enrollment Program is also offered for the aged, blind and disabled to assist in enrolling and receiving insurance programs, such as Medicaid, long term care (for homecare) enrollment, Medicaid recertification, Medicare Savings Program.

Facilitates a smooth transition from the hospital to home for Nassau County children under 18 years of age with mental illness. Provides crisis assessment, individualized planning and family needs assessments. Offers family support by a treatment team to ensure adequate discharge planning and access to community-based services to promote long term symptom management and recovery.

Community Supports and Day Services are designed to provide enriching opportunities for people with developmental disabilities to enhance their social skills and their independence. Programs are responsive to new and changing interests of people receiving supports and their families. The menu of available supports focus on social skills, daily living skills, wellness, technology, music, and the arts. Participants are fully included in their communities through recreation and experiential learning.

Aids individuals with intellectual and developmental disabilities in making choices and finding individualized services. They provide help in navigating through available services including assistance with eligibility determination, service coordination, and advocacy services. Service coordination is also available to children ages birth to 3 years old who have been diagnosed with a developmental disability or delay.

* Aims to reduce the incidence of shootings, killings, and other acts of violence using a public health approach. Identifies and engages high-risk youth in case management Conducts youth outreach Prepares public education campaigns Mediates conflict Provide referrals to local agencies Involves faith leaders Have presence in the streets that neighborhood shootings occur

* Provides coordination of services for adults living with serious mental illness, substance abuse and medical concerns, offers referrals to: Residential Care Coordination. Assertive Community Treatment. AOT services through the SPOA process.

* Provides and coordinates care management. Health Home services are provided through a network of community-based agencies that work with individuals with serious and complex physical health, mental health, and substance use disorders to achieve better health outcomes and reduce cost.

The Office for the Aging offers many services for the senior residents of Steuben County including, but not limited to, information and referral, nutrition through congregate meals, advocacy, support groups, transportation services, caregiver services, case management, and the Bone Builders and Senior Meal Programs. Bone Builders and the Senior Dinner Clubs are overseen by Pro Action of Steuben-Yates.

Office for the Aging Continuing Care provides access to anyone, regardless of age and payment source, to long term care services. Case Managers and Public Health Nurses can assess the clients in their home, the nursing home or in the hospital, establish a realistic plan of care, and provide advice on available funding sources. They may also make suggestions regarding other alternatives such as Adult Day Care, Adult Homes, Assisted Living, Foster Care, Home Health Care, Respite Services, and Nursing Home placement. Office for the Aging / Continuing Care can help arrange for the provision of services, which may include in-home care such as homemaking and personal care, help with paperwork, etc.

Offers case management including advocacy and empowerment, nutrition assistance, and one-on-one volunteer support program to clients and families. Provides referrals related for medical and legal issues, spiritual needs, mental health support, HIV antibody testing, and prevention education programs focusing on: HIV information, prevention, and risk reduction.
