Assists veterans with long-term care needs to stay in their own homes. Design to help qualifying Veterans to hire, train, and supervise care attendants. Personal Care Attendants help with the Veteran’s daily needs of care, provide caregiver respite, and perform light housekeeping.

Provides referrals related for medical and legal issues, spiritual needs, mental health support, HIV antibody testing, and prevention education programs focusing on: HIV information, prevention, and risk reduction.

Office for the Aging Continuing Care provides access to anyone, regardless of age and payment source, to long term care services. Case Managers and Public Health Nurses can assess the clients in their home, the nursing home or in the hospital, establish a realistic plan of care, and provide advice on available funding sources. They may also make suggestions regarding other alternatives such as Adult Day Care, Adult Homes, Assisted Living, Foster Care, Home Health Care, Respite Services, and Nursing Home placement. Office for the Aging / Continuing Care can help arrange for the provision of services, which may include in-home care such as homemaking and personal care, help with paperwork, etc.

* Offers care management to families enrolled in a Children's Health Home. A Children's Health Home is not a physical building or home, but rather a virtual network of support that provides a key point of contact, coordinator of services and compassionate care.

Participants need not be a Family & Children's Services client.

Services include: Individual mental health counseling. Group therapy. Support therapies (Art). Case management. Specialized groups to support reintegration.

* Offers individualized services to persons who are disabled such as: Homemaker basic skills training for caregiver. Parent support group. Companionship and respite services. Family crisis program. Transportation to and from recreation events. Children's day camp, games, sports, supervised exercise. After school wellness program - for school-age children who have high medical or behavioral challenges. Financial Reimbursement for families to assist with costs of caring for a family member with a developmental disability. Behavior Management Services - providers will assess behavior, make referrals and develop a support plan to assist families to manage challenging behaviors in the home.

iCircle Care provides coordinated health and social services to people eighteen (18) years of age or older who are Medicaid eligible and require more than 120 days of long term care supports and services due to chronic illness and/or disability and meet Eligibility Requirements. iCircle Care is dedicated to empowering its members to safely reside in their home or community. A Managed Long-Term Care (MLTC) plan is designed to streamline the delivery of long-term services to people who are chronically ill or disabled and who wish to stay in their homes and communities. These services, such as home care or adult day care are provided through managed long-term care plans approved by the NYS Department of Health.

* Provides non medical personal care, housekeeper chore aide services, case management, information and referral services and supportive counseling to assist individuals to remain living independently at home.

Community Supports and Day Services are designed to provide enriching opportunities for people with developmental disabilities to enhance their social skills and their independence. Develops an Individualized Service Plan (ISP) which outlines what goals the individual hopes to accomplish, the strategies to achieve these goals, and what support they will receive along the way. Plans are evaluated and updated as the individual progresses and to ensure all support and services are meeting the individual's needs. Also assists individuals with services such as future planning, residential planning, recreation, residential habilitation, day habilitation, reimbursement, adaptive equipment, and environmental modifications.
