VIAL OF LIFE | VIAL OF LIFE

The Vial of Life Program is a free program designed for seniors and individuals with serious medical conditions to provide pertinent medical information to emergency personnel who respond to home emergencies. A Vial of Life contains the medical history needed by the responding emergency personnel, such as existing medical conditions, allergies and medications currently being taken. Vial of Life participants complete a Medical Information Form that is then placed on the refrigerator. A Vial of Life decal is then placed on the front door in plain view for emergency personnel to alert them that medical information is located inside the home. Yellow Dot is a free program designed to help first responders provide life-saving medical attention after a crash or other emergency. A Yellow Dot in the rear window of your vehicle will alert first responders that vital medical information is stored in the glove compartment. Yellow dots can also be placed on a front door of a residence and the vital medical information can be in the residence on the refrigerator.

SENIOR SUPPORT PROGRAMS | TOWN OF NEW CASTLE RECREATION AND PARKS DEPARTMENT

Provides a Citizen Assistance Registry to place important medical information that can be located easily in case of an emergency. Also provides daily computerized telephone calls to those living alone or homebound in the community, and if the phone is not answered, a police car is dispatched to check on the individual. May also register for this with the police department, a collaborative program.

FILE OF LIFE KIT | SOMERS SENIOR SERVICES

Call or come by to pick up a free File of Life Kit. The File of Life is a card which contains vital information and is kept on the outside of the resident’s refrigerator in a bright red magnetic pocket, readily available to EMTs when they arrive on an emergency call.The medical card lists the patient’s emergency medical contacts, health problems, medications taken, allergies, recent surgeries and more.A door decal is placed on the outside door to notify arriving EMTs. Also be sure to fill out the compact version which can be carried in a wallet or purse.

Provides support services to homebound seniors, frail elderly or those with disabilities, from the Town of Pound Ridge, to help make their lives safer. The R-U-OK program offers a daily computerized call to participants, to make sure they are doing okay. If there is not a response a police officer is sent to make sure everything is okay. The File for Life provides a magnetized container which holds medical information, placed on the refrigerator, in case of an emergency and is available to anyone responding to help that person.The Loan Closet provides donated medical equipment, such as wheelchairs, walkers, mobile toilets, canes, etc to seniors with limited finances.The Meals on Wheels program offers home delivered meals on a temporary basis.

FIRST RESPONDER RESIDENT ALERT REGISTRY | YORKTOWN POLICE DEPARTMENT

The First Responders Alert Registry is a unique plan to assist Town of Yorktown residents with special needs in emergency situations. The goal of this program is to reduce emotional trauma, fear and conflict when first responders have contact with special needs residents by providing responders with essential information before arriving. The information collected here is confidential and will NOT be available to the public. The information will be held securely by the Yorktown Police Department and only used for emergency response and planning. Residents who want to sign-up for the First Responders Resident Alert Registry can fill out the form linked to https://www.yorktownpd.com/first-responders-resident-alert-registry.html. Residents can also download the department's registration form and submit in person at the Yorktown Police Department Main Desk or by mail.
