Provides supportive services such as non-medical home care, personal emergency response systems, case management, and information and referral services to income eligible seniors aged 60 and older residing in Niagara County.

Provides home care services for those who may need continued care due to age, illness or disability. Services include home nursing, home health aides, personal care, homemaker services, physical, occupational and speech therapy. Tele-health is also available which allows an individual to be monitored by a health professional from the privacy of home. Also offers personal emergency response systems that puts an individual in touch with emergency assistance at the press of a button. Also available is Med-Ready:a medication dispensing unit which helps a patient to take the proper medication at the proper time. This helps to avoid over dosages and medication interactions.

Provides Personal Care Level I and Personal Care Level II services. Level I services consist of assistance with Instrumental Activities of Daily Living (IADLs), such as housekeeping, cooking, and shopping. Level II services consist of assistance with both IADLs and Activities of Daily Living (ADLs), such as dressing, bathing, and moving in and out of a bed, chair, etc. Also provides personal emergency response buttons.

Offers a telephone-based and cell phone emergency response system that links an individual at home to 24-hours a day, seven days a week emergency assistance. The system consists of a home communicator unit attached to the subscriber's telephone, a personal call button worn by the subscriber and the Emergency Response Center located in the Emergency Department.

Personal Emergency Response System (PERS) | Yates County Office for the Aging

Provides electronic equipment which connects frail elderly individuals or people who have disabilities with participating hospitals, paramedics or other sources of emergency assistance. A unit is connected via the telephone line and a pendant is worn by the individual. If the person needs assistance, they press the button on the pendant and the unit dials 911 who will dispatch emergency responders. If the smoke detector is set off, the unit dials 911 and they dispatch the fire department. The system will work if the person is outside within 500 feet of the base unit.
