Offers a deferred loan up to $20,000 to cover the expenses associated with home repair to income eligible homeowners

Provides a group home environment with 24-hour staff supervision with short term, focused skill development in a home-like setting. Services include crisis intervention, rehabilitation counseling, social and living skills development (money management, personal hygiene, food preparation, meal planning and recreation activities), medication management, and community integration.

Provides a one time grant of up to $1,000 if you have been diagnosed with breast cancer and are currently under a physician's care. This grant can be used for such services as house cleaning, meal preparation, transportation expenses, or just a day of relaxation at a local spa.

Provides in-home support services and activities to allow intellectually and/or developmentally disabled persons to live, work, socialize and participate in the community.

Provides a full range of nursing, health aides, and personal care services, from basic medical and daily living needs to more specialized medical services such as wound care, IV therapy, diabetic care, catheter care, cardiac and respiratory assessment, medication management, pain management, and cancer care. Offers a diversion/transfer program for individuals who want to get out of a nursing home and receive care at home. Provides fiscal intermediary services for consumer directed services (also known as CDPAP or CDPAS)

Provides Home Health Aides, Personal Care Aides, Homemakers/Companions, RN's and LPN to the ill, aged and infirm in the privacy and comfort of their home. Services are provided by the hour, day or live-in basis.
