The Residential Repair program provides household maintenance assistance to residents age 60 and over who are homeowners, condominium owners, or apartment renters. The service provides assistance with minor home repairs that do not require the skills of a licensed craftsperson. There is no charge for this service, however, client must purchase any materials required. When work is completed, client is given the opportunity to make a voluntary contribution to the program. Hours of service per resident are limited to 16 hours per fiscal year (April 1 - March 31). WILLCARE provides skilled nursing, home health aides, and homemaker services, including physical and occupational therapy, and nutrition counseling.

Program is available to those individuals who require more support, but no longer need a community residence. Staff members visit on an as needed basis and assist in medication monitoring and additional services. Must meet HUD criteria.

Some services include personal care, case and care management, personal emergency response devices, long-term home care for individuals who are eligible for a nursing home, but choose to stay at home. Home care services are physician ordered and supervised by a registered nurse, and services are rendered by licensed providers who are contracted and monitored the Dept. of Social Services.

Offers Home health Aids (HHA'S), Personal care Aides (PCA'S) and companion care services throughout Nassau County. Services are provided on an hourly basis through private pay, long term care insurance, Office of the Aging, Medicaid Workers Compensation, Managed Long Term care (MLTC), and Mainstream Managed care (MCO) programs.

Provides a 14 passenger DBSC bus to take local senior citizens to and from the senior center.

Offers NYS Licensed Home Care, ranging from skilled nursing care, home health aides, personal care aides to non-medical companion care and homemaker services.

Provides services to chronically ill or physically disabled individuals who have a medical need for help with activities of daily living (ADLs) or skilled nursing services. Services include any of the services provided by a personal care aide (home attendant), home health aide, or nurse. Recipients have flexibility and freedom in choosing their caregivers.

Home-aid provides assistance with light housekeeping, errands and shopping for home-bound elderly who are unable to perform these tasks themselves. Assessment and case management are provided. Self-referrals accepted.

Provide supervised living residences in the community for small groups of young adults, which includes community integration and participation, social skill development, and heavy involvement in community-based leisure-time activities. Opportunities for meaningful volunteer work in the community, supported work and competitive employment help promote the development of independent living skills and job skills.

This service replaces the Targeted Case Management Services. This agency contracts with Adirondack Health Institute (AHI) and provides linkage and supports to assist individuals with their mental and physical health care needs. OMH funded and contract with Health Home and DOH.

The Town of Newburgh Friends of Seniors is a program that is affiliated with the Town of Newburgh, the Orange County Office for the Aging and Jewish Family Service of Orange County. We train and screen volunteers to take our seniors to doctor appointments, shopping or just a friendly visit to a homebound senior.

Our Senior Independence Program matches local volunteers with older individuals and adults with disabilities to help them continue living independently and offer simple gestures that help them feel cared for. From providing a ride to the doctor, helping with light household chores, helping with grocery shopping, or just stopping by for a friendly visit, our volunteers help individuals be more connected to their communities and have the help they need to continue living in their home.

The Empower New York program offers no-cost energy efficiency services to low-income (i.e. HEAP eligible) homeowners and renters.

Delivers planned and coordinated services to meet each patient's specific home health care needs including: registered nurses, home health aides, personal care aides, live-ins, and companions. Provides total case management including contacting the patient's physician, arranging for equipment needs and works closely with family to assure the best care for every patient.

Provides assistance, to people recovering from mental illness or substance abuse, find and maintain a permanent home.
