Provides outpatient mental health services for individuals ages 5 years and older.

Provides a care coordination program that assists individuals that are residing in a community setting or are transitioning to live in a community setting. Services include behavioral, educational, and social health support, transportation, and housing support. On-call services are available 24/7.

* Provides mental health and crisis services which include: Crisis intervention/stabilization. Mental health assessments. Rapid linkage and/or re-linkage to treatment providers. Interim/bridge mental health treatment. Mental health transport. Connections to treatment providers and support systems in the community. Referrals to non-mental health services.

Collaborative team working together to improve outpatient mental health services for adults, children and families and to decrease avoidable hospital admissions and re-admissions.

* Offers individual, group, and family therapy for adults, children, and families. * Walk in hours for in person crisis intervention.

Provides an outpatient/inpatient clinic to help adults and children struggling with mental or emotional problems. Services include evaluation/screening, diagnostic and treatment planning services for people who are experiencing acute or chronic psychiatric problems, individual, group and family therapy, also crisis intervention.

Provides individual and group bereavement counseling, child abuse counseling, crisis intervention to people experiencing stressful situations such as death of a family member, separation or divorce, severe accident or illness, surgery, rape, child abuse, loss of job or home.

Provides an Extended School Day and School Violence Prevention program. Weekly clubs offer literacy activities through hands-on programming. School Violence Prevention provides crisis-intervention counseling and support groups to target protective assets and risk factors.

Provides mental health crisis intervention and support services within the home or community. Services include immediate access to treatment services designed to stabilize crisis situations, enhancing skills and knowledge to reduce environmental and social stressors, and connections to programs and services.

Provides inpatient and outpatient mental health assessments and treatment for depression, psychiatric disorders, etc.

* Responds in person to mental health, domestic violence and other related crises. Intakes are accepted on a case by case basis and determined by the availability of the team.

The YES program is designed to address truancy and at-risk behaviors by working one-on-one with youth through positive communication and mentorship, and promoting healthy futures for Chenango County youth and their families.

Provides an in-home crisis intervention service to families with youth ages 5 to 17 years for four to six weeks to assist with crisis management, improve family functioning, support families with tools to manage crisis situations, and by building upon the families' natural support systems to create more safety, and stability within the home.

* Provides immediate emergency assistance to households in crisis including: Food bags. Personal Hygiene Bag. Prescription Assistance. Bus Passes (when available). Stranded Traveler Assistance. Clothing Assistance. Diaper Assistance. Vouchers to area supermarkets for food and personal items. Advocacy and referrals to outside agencies. Crisis assessment and management. Bilingual Services. Registration for Salvation Army Programs (Project Bundle Up, Long Point Camp, Christmas Assistance, etc.)

The Intake Department facilitates access to a short-term Crisis Respite intervention for children and their caregivers due to a child’s behavioral health, developmental, or medical crisis or trauma, including acutely challenging emotional or medical crisis in which the child is unable to manage without intensive assistance and support.

Provides counseling for various issues, including career counseling, through individual, family and group methods.

Provides support services for women who have been victims of domestic violence, services include support groups, crisis intervention, case management,community outreach and education, information and referral and emergency transportation.
