A committee designed to link families who may require more intensive mental health support to services such as Care Coordination, Home-Based Crisis Intervention, Mental Health Respite Program, Home and Community Based Services, Family Support, and OMH services such as Residential Treatment Facility, Community Residence, Children's Psychiatric Center, or mobile crisis programs. Families are encouraged to invite schools and others working with their child to attend a meeting; approach is strength-based and constructive; jointly crafted written plans are provided to families at the conclusion of the meeting. Committee is made up of representatives from the following organizations: Wyoming County Department of Mental Health, Social Services, Probation, and Youth Bureau; Spectrum Health & Human Services (Certified Community Behavioral Health Clinic Services including Mental Health Counseling and Chemical Dependency Services), Hillside Children's Center, Wyoming County Community Hospital (Crisis Outreach), GLOW Family Support, Clarity Wellness Community, New Directions and Western New York Children's Psychiatric Center.

Behavioral Health Services North provides adult, child and family clinical services at our Center for Well-Being – a community mental health clinic.

Provides face to face assessment, outreach and support, immediate stabilization of acute symptoms of mental illness, alcohol and drug abuse and emotional distress.

Offers evaluation and counseling for children, adults, and families struggling with emotional issues. Also provides on-site sexual abuse counseling.

Provides a mental health assessment and links them to treatment, for juvenile delinquents and their families, in an effort to avert out-of-home placement.

Provides mental health care facilities, behavioral health, crisis intervention and evaluations to help those struggling with emotional or mental illness.

Offers outpatient programs and a wide range of services by mental health professionals. Our staff includes psychiatrists, psychologists, licensed clinical social workers and activities therapists. Patients can join individual, group and family psychotherapy, and we offer specialized services for depression, anxiety and childhood emotional disorders. Specific programs include: Adult Psychotherapy Program for individual or group therapy; Family and Children's Program offering individual, group and family psychotherapy for children and their families and Options for more intensive care at South Nassau: Partial Hospitalization Program, five days weekly; Intensive Outpatient Program, three days weekly and Inpatient Behavioral Health Unit. *Provides crisis intervention, client/situation stabilization services face-to-face to divert non-lethal individuals from the county's psychiatric emergency departments as well as to find, support and link individuals to mental health services. Some services include:

• Crisis hotline.
• Mental health assessments.
• Crisis de-escalation and de-briefing.
• Linkage and follow-up to services.
• Alerts to identify and assist those at an increased risk.
• Coordination with schools, medical/ mental health providers and other agencies to provide crisis intervention and to help monitor clients who are at risk.

* Provides Open Access, a 24/7 response to those individuals living with addiction.

* Provides services in partnership with the Rescue Mission of Utica.

Provides wrap around services to individuals who have been diagnosed with a severe and persistent mental illness. Assists with service planning and coordination, housing supports, money management, family education, substance abuse treatment, vocational support, mental and physical health treatment, and community referrals to offer support, rehabilitation, and treatment within the community.

Provides psychiatric outpatient services to clients ages 18 and older.

Provides assessment, referral and treatment of children suffering from mental illness and addiction issues.

This service replaces the Targeted Case Management Services. This agency contracts with Adirondack Health Institute (AHI) and provides linkage and supports to assist individuals with their mental and physical health care needs. OMH funded and contract with Health Home and DOH.

The Adult Single Point of Access (A-SPOA) is a process to identify those adults with a mental illness who are most in need of services and to provide access to supports and services such as care management, assertive community treatment (ACT), OMH licensed housing and supportive housing. The A-SPOA Coordinator works with referral sources, families, and hospitals to ensure a smooth transition to appropriate care.

Provides a team approach to community supports, linkage to assessments, evaluations, and service providers as appropriate.

A centralized intake process for referrals to high-intensity mental health services for children and adults who have been diagnosed with a serious mental illness that interferes with their ability to function in the community, home, school, and/or work. The overall goals of the mental health services include fostering independence and improving quality of life. The purpose of SPOA is to manage access to high-intensity mental health services, provide a forum for improved collaboration among community service providers, and identify and promote community-based alternatives to residential treatment and psychiatric hospitalization.

Care Coordination/Case Management Services are intended for the high need consumer who has had difficulty maintaining linkages to mental health services and other important supports, such as benefits, housing, advocacy, health care, peer services, or vocational services. SPOA is a uniform referral process which assists the referring party to determine the appropriate level of care coordination services. After the referral is made it is assessed and a determination is made as to the acceptance of the initial referral or referral to another program which may better meet the needs of the individual. Levels of care include: transition case management, care coordination, assertive community treatment, and assisted outpatient treatment.

Provides linkage to services for children, birth to 17 years who are experiencing a severe mental health or behavioral difficulties and their families.

Provides mental health assessments/crisis intervention to individuals struggling with emotional or mental difficulties, including assisting DSS or family court with home assessments and therapeutic intervention for adoption and foster care families and for on matters involving custody, visitation, child abuse/neglect, and PINS (Persons In Need of Supervision).

Provides mental health assessments and evaluations, along with treatment through individual, group or family therapy, including adolescent and child counseling, and an anger management program.

Provides Care Coordination, referral, monitoring, and access point for children and adults with serious mental illness, emotional problems, and/or behavioral problems who need more intensive support and care coordination.

Program is for individuals, families, and service providers seeking Assertive Community Treatment (ACT), care management, residential or housing services for adults and children living with mental health illness/emotional disturbances. Assesses the strengths and needs of those referred and then creates a plan for linkage with the most appropriate services. Coordinates with Health Homes for most appropriate Care Management service as appropriate.

Provides outpatient mental health services to clients who seek help for depression, anxiety, suicidal feelings, and psychiatric illness.

Assists children and their families with a multidisciplinary team to provide support during and after treatment. Provides services to children ages 5 to 18 years, who have problems that range from mild behavior disorders to psychiatric illnesses.
