Offers a psycho-social clubhouse, open six days a week, which helps its members to develop vocational, educational, housing and social opportunities. Participants have access to computers, educational scholarships, assistance with writing resumes, work incentives and benefits as well as placements in transitional and independent employment opportunities.

An Office of Mental Health (OMH) funded Recovery Center offering services for adults with mental health conditions. Services include employment support, benefits advisement, parenting classes and support, financial literacy development, educational services, volunteer opportunities, meals, and some social activities. Provides a complete continuum of residential care for mentally ill adults who require assistance to live successfully in the community. Traditional housing programs certified by the NYS Office of Mental Health offer placement in community residences and scattered site apartments. Permanent housing is available in scattered site apartments in Yonkers and Mount Vernon.

Offers a 30-day psychiatric day program which includes individual, group and family counseling, recreational therapy, therapeutic socialization activities, emergency stabilization, medication, and a special education through Western Suffolk BOCES.

In this supervised phase of the program, residents live in group homes. Trained counselors are on site 24 hours a day for support and assistance at these homes. A Young Adult Program is also offered to individuals ages 18-25. Residents who live in the supervised sites receive counseling, medication management, assistance with cooking and cleaning, food shopping, money management, and they participate in rehabilitative activities. Weekly community meetings give residents a chance to take an active role in planning house activities and to discuss any concerns they have about the home. The emphasis in this phase is to support the resident in acquiring basic living skills.

Offers performances by an improvisational theater group which explores the myths and realities of mental illness. Features vignettes about family conflicts, stigmas, divorce, troubled teens, group homes, living with the mentally ill, suicide, homeless and more. Discussions are led by a professional mental health facilitator. Provides outpatient psychiatric treatment and follow-up care after hospitalization. Monitors medications and compliance with treatment programs, and provides therapy as needed. Also offers psychological therapy. Employs services of psychiatrists, physicians, and other medical staff with special expertise in the care of geriatric psychiatric patients and of patients with co-morbidities.

A residential home in Elmont for adults with persistent mental illness. Provides a 24 hour community residence that serves men and women 18-65 years of age, who suffer from mental illness. Services include daily living skills training, socialization skills, medication management, substance abuse prevention, assertiveness/self-advocacy training, case management, health services, and parenting education. Additional programs focus on community integration, vocational/educational skills development and rehabilitation counseling.

Provides an array of rehabilitation and recovery services for adults with mental illness which works to assist individuals in acquiring and maintaining skills and supports to live successfully in their communities. Services include wellness self management, financial management, leisure planning, reduce risk of relapse, employment support, medication management and more.

Provides short-term, home based services to families with children who are in or were recently released from in-patient psychiatric facilities. Provides support, counseling, training, and life-skills for people with mental health and substance use needs. Services are provided at home, in the community, or through telehealth and can in addition to other behavioral health services you are already receiving. Services include: Peer Support Services, Psychosocial Rehabilitation, Counseling, Family Support & Training, and Education and Job Support Services.

The Mobile Crisis Team is a team of Licensed Mental Health Professionals, Support Staff, and Peer / Family Advocates available to any Chautauqua County resident experiencing a mental health crisis or distress during non-business hours. The Mobile Crisis Team provides access to Licensed Mental Health Professionals for telephone and/or face to face mental health assessment and crisis intervention, resolution of crisis in the least restrictive setting and least intrusive manner possible, prevention of unnecessary emergency department (ED) visits, and linkage to appropriate community mental health resources for ongoing treatment and stabilization. Monitoring services are also provided for up to 72 hours for individuals who have been released from mental health hospital stays. Also offers mental health training for community organizations and mental health professionals.

A social and recreational program providing adults recovering from psychiatric illness with activities and programs to develop the strength and confidence necessary to integrate more successfully into the community. Groups provide social, pre-vocational services, skills, education and information and assist members in developing coping skills.

Includes three ten-bed homes which serve as transitional housing for persons with psychiatric disabilities, and for individuals with a dual diagnosis of mental illness and intellectual disabilities. A Supported Housing Apartment program is available for those who are ready to move on to a more independent living situation.

Provides meaningful volunteer opportunities for people who are in recovery from mental illness. The companions are placed in approximately 25 locations including adult homes, nursing homes and also private homes and spend time socializing, sharing wellness tools and providing companionship. This program not only offers volunteer opportunities but it is a pre-vocational program which offers weekly trainings and skill building activities to enhance abilities and readiness for gainful employment.

Offers a variety of home care services based on the needs of the patient and their families. There are health aides for day/night hours and live-in aides for continuous care. Also provides personal care aides for meal, laundry, shopping, and errand services, and nursing home care for those who need more involved medical care. Also provides short term in-home respite care for family caregivers, and hospice care for individuals who are dying or are at the end stage of an illness.. Offers specialized medical care for Alzheimer's, Dementia, Stroke, HIV/AIDS, Diabetes, Psychiatric aftercare, Brain Injuries, and Postpartum care.

Assists school age students from Nassau and Suffolk counties to avert hospitalization for severe emotional/psychiatric problems and students with psychiatric problems who are on the Autism Spectrum. The IDT Program is also a therapeutic program to help a youngster adjust to the community following a period of inpatient psychiatric hospitalization or a short term placement for youngsters leaving psychiatric hospitalization en route to day treatment or other specialized school-based services. Instructional staff provided by the Special Education Division of Western Suffolk BOCES in concert with professionals funded by the Office of Mental Health/Sagamore Children's Psychiatric Center, provide intensive intervention to students on an extended day basis. The program also works with pupils who have been in a day treatment or other intensive school placement and have not been successful.

Rehabilitative Social Club Program provides socialization opportunities for adults residing in Orange County with mental illness. Transportation is available for program participants living in the Middletown area.

Provides 24 hour group home settings and more independent transitional scattered site residences for adults with a diagnosis of mental illness. Staff provides supportive assistance and instruction in daily living skills and use of community resources.

Offers a socialization and recovery program to Nassau County residents with severe mental illness, ages 18 and older. Provides opportunities in a "club house setting" for individuals to improve socialization skills, physical, emotional and spiritual health and decision making, reduce symptoms, maintain sobriety, and learn technology skills. Provides 24 hour supervision at local community sites for adults with mental illness. Services provided at each site are restorative services, symptom management, community living skills, information and referral, crisis management, medication education and monitoring, and community integration. Staff provides supportive assistance and instruction in daily living skills and use of community resources. There are ten residences located in Nassau and Suffolk County.
