The Mobile Crisis Team is a team of Licensed Mental Health Professionals, Support Staff, and Peer / Family Advocates available to any Chautauqua County resident experiencing a mental health crisis or distress during non-business hours. The Mobile Crisis Team provides access to Licensed Mental Health Professionals for telephone and/or face to face mental health assessment and crisis intervention, resolution of crisis in the least restrictive setting and least intrusive manner possible, prevention of unnecessary emergency department (ED) visits, and linkage to appropriate community mental health resources for ongoing treatment and stabilization. Monitoring services are also provided for up to 72 hours for individuals who have been released from mental health hospital stays. Also offers mental health training for community organizations and mental health professionals.

Assists individuals with functional mental health-related disabilities to successfully identify, pursue, and achieve their life goals at home, work, school, and in the community. Program is provided in collaboration with Capabilities and is A NYS OMH-licensed comprehensive PROS Program. Clients receive Community Rehabilitation and Support (CRS) services; Intensive Rehabilitation (IR) services; Ongoing Rehabilitation Support (ORS) services; and Clinical Treatment (CT).

Provides a full range of assessment and individual, family and group mental health therapies to adults. Included are dual recovery services that are provided in conjunction with area substance abuse treatment providers for individuals diagnosed with mental health and Substance Use Disorders (SUD). Also provides trauma care for individuals having mental health difficulties related to current and/or past abuse experiences, including victims, survivors, and offenders. Offers an Injection Clinic.

Provides housing assistance and case management services to individuals and families diagnosed with mental illness, substance abuse and other qualifying criteria.

Provides supported housing opportunities for individuals recovering from mental illness needing a stable living environment. Individuals are provided the necessary support to live independently. Offers three community residences, with one focusing on individuals with both a mental illness and a chemical addiction (dual diagnosis), with 24 hour supervision that are licensed by the New York State Mental Health.

Provides housing and supportive services aimed at assisting clients who have a psychiatric disability to maintain themselves in the community. Serves Nassau County residents 18 and over with a primary psychiatric diagnosis. For certain locations, individuals must also meet HUD requirements for homelessness. Referral required. OASAS program-primary substance abuse diagnosis and subsequent hospitalization in addition to homelessness.

Provides learning experiences where individuals can develop strengths and skills that contribute to an individualâs employability in the community. Services include teaching concepts such as compliance, attendance, task completion, problem solving, safety, wellness while at work, socialization at work, communication, accepting supervision, personal appearance and appropriate work attire; goal setting, work ethics, and work tolerance.

Helps to alleviate the feelings of isolation and loneliness that people feel due to their illness by providing an opportunity for adults suffering from schizophrenia, bi-polar disorder, depression or other personality or mood disorders to develop friendships and social skills. Referral required.

An Office of Mental Health (OMH) funded Recovery Center offering services for adults with mental health conditions. Services include employment support, benefits advisement, parenting classes and support, financial literacy development, educational services, volunteer opportunities, meals, and some social activities. Individualized, same day services are available. Certified Benefits Advisors on staff. Certified Employment Network offers Ticket to Work services for SSI/SSDI beneficiaries. ##REENTRY##

Provides a family living situation for adults who need supervision and care. Caring people open their homes to adults that are mentally or physically impaired who do not require the skilled care of a nursing facility. A request to become a Family-Type Homes for Adults operator or to refer an interested adult may be made directly to the Office of Adult Services.

Provides support, counseling, training, and life-skills for people with mental health and substance use needs. Services are provided at home, in the community, or through telehealth and can in addition to other behavioral health services you are already receiving. Services include: Peer Support Services, Psychosocial Rehabilitation, Counseling, Family Support & Training, and Education and Job Support Services.

A 24-hour state-operated supervised transitional community residence with 14 beds that offers support and skills training to individuals with mental illness.

A 24-hour state operated supervised residential care center for adults. Offers support skills training to individuals with a diagnosis of mental illness who are preparing for more independent living environments.

We offer a comprehensive array of treatment, rehabilitation and clinical services that are customized to meet the needs of each participant. Free lunch included while at the program.

Residents choose their own apartment, receive funding for furnishings and security deposits, and may choose to live alone or have the option of selecting an apartment-mate(s) whom have mental health diagnosis. Staff visit residents at least once a month or more often if necessary.

Provides an array of rehabilitation and recovery services for adults with mental illness which works to assist individuals in acquiring and maintaining skills and supports to live successfully in their communities. Services include wellness self management, financial management, leisure planning, reduce risk of relapse, employment support, medication management and more.

A drop-in center, for those who are program participants, to achieve competitive work skills, access computers, socialize with others, play music or sing, make deli type lunch, watch TV or do arts and crafts.

Offers a socialization and recovery program to Nassau County residents with severe mental illness, ages 18 and older. Provides opportunities in a "club house setting" for individuals to improve socialization skills, physical, emotional and spiritual health and decision making, reduce symptoms, maintain sobriety, and learn technology skills. Provides 24 hour supervision at local community sites for adults with mental illness. Services provided at each site are restorative services, symptom management, community living skills, information and referral, crisis management, medication education and monitoring, and community integration. Staff provides supportive assistance and instruction in daily living skills and use of community resources. There are ten residences located in Nassau and Suffolk County. Transportation provided to local day treatment services.

Offers a comprehensive recovery-oriented program for individuals with severe and persistent mental illness. The goal of the program is to integrate treatment, support and rehabilitation in a manner that facilitates the individual's recovery.

Provides outpatient mental health services, such as mental health screening, individual, group, child/adolescent counseling, geriatric services, including after care support.

In this supervised phase of the program, residents live in group homes. Trained counselors are on site 24 hours a day for support and assistance at these homes. A Young Adult Program is also offered to individuals ages 18-25. Residents who live in the supervised sites receive counseling, medication management, assistance with cooking and cleaning, food shopping, money management, and they participate in rehabilitative activities. Weekly community meetings give residents a chance to take an active role in planning house activities and to discuss any concerns they have about the home. The emphasis in this phase is to support the resident in acquiring basic living skills.

Provides services for Suffolk County residents age 18 plus who are in recovery from mental illness. Call for details.

Peer facilitated psychosocial club for seriously and persistently mentally ill adults focused on socialization and empowerment. Members are invited to drop in for any of the gatherings. Offerings include but are not limited to: participation in community events, games, videos and crafts in a relaxed atmosphere designed to encourage interaction and reduce isolation. Transportation assistance is available when possible.

Federation of organizations offers over 750 permanent, safe and affordable residential placements in our supportive scattered-site Housing program. In this program, individuals with disabilities live in apartments or houses throughout Nassau, Suffolk, Queens, Brooklyn and the Bronx. We provide specialized supportive scattered-site housing placements that target vulnerable populations such as veterans, shelter-homeless individuals transitioning out of adult homes, state psychiatric centers, prisons and local hospitals.
