Participants have access to computers, educational scholarships, assistance with writing resumes, work incentives and benefits as well as placements in transitional and independent employment opportunities.

Provides a complete continuum of residential care for mentally ill adults who require assistance to live successfully in the community. Traditional housing programs certified by the NYS Office of Mental Health offer placement in community residences and scattered site apartments. Permanent housing is available in scattered site apartments in Yonkers and Mount Vernon.
Offers a 30-day psychiatric day program which includes individual, group and family counseling, recreational therapy, therapeutic socialization activities, emergency stabilization, medication, and a special education through Western Suffolk BOCES.

In this supervised phase of the program, residents live in group homes. Trained counselors are on site 24 hours a day for support and assistance at these homes. A Young Adult Program is also offered to individuals ages 18-25. Residents who live in the supervised sites receive counseling, medication management, assistance with cooking and cleaning, food shopping, money management, and they participate in rehabilitative activities. Weekly community meetings give residents a chance to take an active role in planning house activities and to discuss any concerns they have about the home. The emphasis in this phase is to support the resident in acquiring basic living skills.

Offers performances by an improvisational theater group which explores the myths and realities of mental illness. Features vignettes about family conflicts, stigmas, divorce, troubled teens, group homes, living with the mentally ill, suicide, homeless and more. The Mobile Crisis Team is a team of Licensed Mental Health Professionals, Support Staff, and Peer / Family Advocates available to any Chautauqua County resident experiencing a mental health crisis or distress during non-business hours. The Mobile Crisis Team provides access to Licensed Mental Health Professionals for telephone and/or face to face mental health assessment and crisis intervention, resolution of crisis in the least restrictive setting and least intrusive manner possible, prevention of unnecessary emergency department (ED) visits, and linkage to appropriate community mental health resources for ongoing treatment and stabilization. Monitoring services are also provided for up to 72 hours for individuals who have been released from mental health hospital stays. Also offers mental health training for community organizations and mental health professionals.

The companions are placed in approximately 25 locations including adult homes, nursing homes and also private homes and spend time socializing, sharing wellness tools and providing companionship. This program not only offers volunteer opportunities but it is a pre-vocational program which offers weekly trainings and skill building activities to enhance abilities and readiness for gainful employment.

Offers a variety of home care services based on the needs of the patient and their families. There are health aides for day/night hours and live-in aides for continuous care. Also provides personal care aides for meal, laundry, shopping, and errand services, and nursing home care for those who need more involved medical care. Also provides short term in-home respite care for family caregivers, and hospice care for individuals who are dying or are at the end stage of an illness.. Assists school age students from Nassau and Suffolk counties to avert hospitalization for severe emotional/psychiatric problems and students with psychiatric problems who are on the Autism Spectrum. The IDT Program is also a therapeutic program to help a youngster adjust to the community following a period of inpatient psychiatric hospitalization or a short term placement for youngsters leaving psychiatric hospitalization en route to day treatment or other specialized school-based services. Instructional staff provided by the Special Education Division of Western Suffolk BOCES in concert with professionals funded by the Office of Mental Health/Sagamore Children's Psychiatric Center, provide intensive intervention to students on an extended day basis. The program also works with pupils who have been in a day treatment or other intensive school placement and have not been successful.
