The Mobile Crisis Team is a team of Licensed Mental Health Professionals, Support Staff, and Peer / Family Advocates available to any Chautauqua County resident experiencing a mental health crisis or distress during non-business hours. The Mobile Crisis Team provides access to Licensed Mental Health Professionals for telephone and/or face to face mental health assessment and crisis intervention, resolution of crisis in the least restrictive setting and least intrusive manner possible, prevention of unnecessary emergency department (ED) visits, and linkage to appropriate community mental health resources for ongoing treatment and stabilization. Monitoring services are also provided for up to 72 hours for individuals who have been released from mental health hospital stays. Also offers mental health training for community organizations and mental health professionals.

Assists individuals with functional mental health-related disabilities to successfully identify, pursue, and achieve their life goals at home, work, school, and in the community. Program is provided in collaboration with Capabilities and is A NYS OMH-licensed comprehensive PROS Program. Clients receive Community Rehabilitation and Support (CRS) services; Intensive Rehabilitation (IR) services; Ongoing Rehabilitation Support (ORS) services; and Clinical Treatment (CT).

Provides a full range of assessment and individual, family and group mental health therapies to adults. Included are dual recovery services that are provided in conjunction with area substance abuse treatment providers for individuals diagnosed with mental health and Substance Use Disorders (SUD). Also provides trauma care for individuals having mental health difficulties related to current and/or past abuse experiences, including victims, survivors, and offenders. Offers an Injection Clinic.

Provides outpatient mental health services, such as mental health screening, individual, group, child/adolescent counseling, geriatric services, including after care support.

Provides seriously and persistently mentally ill (SPMI) and seriously emotionally disturbed (SED) individuals who have been discharged from a residential or inpatient facility on-going treatment and supportive services including developing and maintaining viable living, working and social situations within the community.

Offers a variety of home care services based on the needs of the patient and their families. There are health aides for day/night hours and live-in aides for continuous care. Also provides personal care aides for meal, laundry, shopping, and errand services, and nursing home care for those who need more involved medical care. Also provides short term in-home respite care for family caregivers, and hospice care for individuals who are dying or are at the end stage of an illness.. Offers specialized medical care for Alzheimer's, Dementia, Stroke, HIV/AIDS, Diabetes, Psychiatric aftercare, Brain Injuries, and Postpartum care.

Assists in transitioning individuals from long-term care at state psychiatric facilities.

Provides a safe space to support individuals in all stages of recovery. Offers various programs, meetings, and special events that are inclusive to the entire recovery community. The purpose is to help the individual and their support network develop the skills they require to maintain a successful recovery. This is a drop-in center and individuals are welcome and encouraged to drop in at any time during hours of operation.

Houses an adult outpatient clinic which offers comprehensive mental health services to individuals with a qualifying DSM-IV diagnosis. Self-referrals accepted.

Provides short-term crisis support in order to avert an unnecessary psychiatric hospitalization, prevent a lengthy hospitalization, and/or provide step down support following hospitalization prior to return to independent living status or while awaiting alternative community placement.

Offers two certified mental health community residence for adults who need assistance preparing for successful community living.

Psychiatric Aftercare Clinic for clients who have previously been inpatient.

Provides transitional housing for those re-entering the community from inpatient hospitalization and/or for those who have not benefited from traditional residential or supportive housing programs. Provides case management, information and referral, transportation, benefit advisement, on-call and after hours support, advocacy, and support groups via a multi-unit apartment building and the drop-in center. Offers short-term respite with goal of preventing crisis and hospital diversion.

Provides short-term, home based services to families with children who are in or were recently released from in-patient psychiatric facilities. Helps to prevent rehospitalization, enhance family functioning, improve child/parent relationships, and promote self sufficiency. Call for more information.

Offers an array of services designed to assist individuals recovering from mental health disorders in setting goals and developing the skills and accessing the supports necessary to choose, obtain and maintain life roles. Also provides clinical treatment and rehabilitation services.
