Matt Urban Hope House Apartments

Affordable housing apartments for individuals and families who are homeless or at risk of homelessness with a mental health disability. Apartments are located in the Fillmore Council District of the City of Buffalo. Community consists of 27 units: 13 studio, 10 one-bedroom, and 4 two-bedroom apartments. The 4 two-bedroom apartments are dedicated to families. Amenities include professional on-site management, handicapped accessible and adaptable apartments, close proximity to public transportation, smoke free living, a community room, on-site laundry facilities, and an on-site community center.

Offers comprehensive services to children and families, which promotes literacy skills and social, mental, emotional, and physical needs. Parents are the cornerstone of the Head Start Program. A center specializing in Alzheimer's and related dementia day care. Provides social model adult day care, socialization, hot lunch, breakfast and afternoon snack. Services include small group crafts, exercises, field trips, personal care in activities of daily life, ongoing care plans, information and referral, transportation within a 15 mile range, and wheelchair transportation. Inter-generational program with on-site child care offered. Alzheimer's support group held the 3rd Thursday of each month, 7pm-9pm.

Hearts & Hands - Faith In Action, Inc. (Service Group)

Hearts and Hands is a volunteer based non-profit organization that provides free escorted transportation, companionship, and in-home supports for older adults in communities throughout Erie and Eastern Niagara Counties. Our care recipients are often individuals who stopped driving due to age-related medical conditions or for financial reasons. Recipients may also need assistance at home with light yard work and home maintenance, or companionship to renew and refresh their caregiver. Our elderly neighbors are often lonely, isolated, and in need of friendly human interaction. In response to a recent community assessment and a national assessment from AARP, Hearts and Hands has designed our program to address social isolation through all three avenues of service: transportation, household supports, and companionship. Hearts and Hands volunteers may be the only people our care receivers see during a week, and their company is, in and of itself, a bright spot in daily life. Hearts and Hands is a best-efforts program. Services are provided by volunteers who give their time as they have it available.
