Provides limited supplementary financial assistance to help with home health care, child care, and transportation to and from treatment. Additionally, helps people with cancer overcome financial access and treatment barriers by assisting them with co-payments for their prescribed treatments. Basic family monthly income and application form must be filled out.

CARE MANAGEMENT | HCR HOME CARE - PLATTSBURGH OFFICE

Care Management services for Adults and Children who have physical and/or mental health diagnosis. Care Managers will assist with scheduling appointments, arranging for transportation and will attend appointments at the clients request to help advocate for the clients wants and needs. A Care Manager for a child will work with the parents or guardian to ensure a child is safe, healthy and well cared for. We will work with providers including the schools to ensure a child's voice is heard and goals are developed to meet the child's needs.

Individuals having difficulty acquiring adequate transportation in Brookhaven Town or have questions regarding available transportation should contact the Division of Mobility.

Provides self-care training based on the needs and abilities of adults with developmental or acquired disabilities. Training can focus on daily living skills such as bathing, personal hygiene, dressing, meal preparation, eating, household tasks, laundry, transportation, and shopping. Training can be done in a group setting or at home.

Liberty Center for Youth

A free, daily after-school program for students ages 9-16 years old. Offers a computer/tech room with 10 computers as well as 10 tablets that youth may use for homework or recreational purposes. Provides supervised game time in the gym as well as a game room featuring air hockey, pool, ping pong, and foosball. There is a cafeteria/program space used for a daily snack provided by the Batavia City School District. Activities include clubs, open gym and sports, active games, socialization and free choice, homework help, mentoring/tutoring, computers/game systems, community involvement, and enrichment activities. Transportation to the center is provided by the school; however, parents must arrange this with the school.

Provides safe settings, Kindergarten readiness skills, classroom and home-based experience, hot nutritious meals, health screening, community referrals, transportation, non-violent problem solving skills and day care referrals for children and siblings.

Recycle a Bicycle - GObike Buffalo

After school program that teaches students bicycle maintenance and safety. Students also learn about the environmental benefits of non-motorized transportation. Each student is given the opportunity to fix and rebuild a previously used bicycle. At the conclusion of the program, the students ride on their bikes and can then take them home.

Taxpayer Advocate Service - Internal Revenue Service

An independent organization within the IRS led by the National Taxpayer Advocate that helps individuals and business taxpayers resolve problems with the IRS by: ensuring that taxpayer problems not resolved through normal IRS channels are promptly and impartially handled; assisting taxpayers who are facing hardships; identifying issues that impact taxpayer rights, increase taxpayer burden or otherwise create problems for taxpayers and bringing these issues to the attention of the IRS management; and recommending administrative and legislative changes. Situations where the TAS may help include: IRS actions preventing you from providing for necessities such as housing, transportation or food; IRS actions preventing you from being able to meet basic expenses such as payroll, if you are a business owner; delays of more than 30 days to resolve a tax related problem or not receiving a response by the date promised. Low Income Taxpayer Clinic provides representation in federal tax controversies with the IRS free of charge or for a nominal fee. Clinics also provide tax education and outreach for taxpayers who speak English as a second language.

A comprehensive managed long term care program for older adults who wish to live at home but need assistance in order to maintain health, complete day to day activities, and remain safe at home for as long as possible. Primary and specialty medical care, adult day center, meals, prescriptions, hospitalization, and emergency services provided. In-home support, physical and occupational therapy, and transportation are also available.

Baby Needs Program at Buffalo River Food Pantry

Food pantry located at Old First Ward Community Center. Provides bags of groceries. All clients are eligible for pantry bags every other week as well as holiday food baskets. In an emergency, can provide emergency food one time to anyone with a referral or a request for service; however, must register as a client for continuing service.

Federally Qualified Health Center (FQHC) offering pediatrics, internal medicine/family medicine, OB/GYN, pediatric and adult dental services, podiatry, social work, nutritional counseling, medications for opioid use disorder, and pharmacy with home delivery. Serves all populations with special focus on the medically under-insured, uninsured, and those experiencing homelessness. Also provides onsite lab, ultrasound, STD testing, COVID-19 testing and vaccines, health education/groups, blood pressure screening, and free walk-in pregnancy testing. Provides transportation assistance to help clients get to appointments and referral assistance. Participates in 340B drug discount program and has onsite enrollers for the New York State of Health insurance marketplace. Hospital affiliations: Sisters Hospital and the John R.
