Affordable housing apartments for individuals and families who are homeless or at risk of homelessness with a mental health disability. Apartments are located in the Fillmore Council District of the City of Buffalo. Community consists of 27 units: 13 studio, 10 one-bedroom, and 4 two-bedroom apartments. The 4 two-bedroom apartments are dedicated to families. Amenities include professional on-site management, handicapped accessible and adaptable apartments, close proximity to public transportation, smoke free living, a community room, on-site laundry facilities, and an on-site community center.

Description: Federally Qualified Health Center (FQHC) offering pediatrics, internal medicine/family medicine, OB/GYN, pediatric and adult dental services, podiatry, social work, nutritional counseling, medications for opioid use disorder, vision care, and pharmacy with home delivery. Serves all populations with special focus on the medically under-insured, uninsured, and those experiencing homelessness. Also provides onsite lab, ultrasound, STD testing, COVID-19 testing and vaccines, health education/groups, blood pressure screening, and free walk-in pregnancy testing. Provides transportation assistance to help clients get to appointments and referral assistance. Participates in 340B drug discount program and has onsite enrollers for the New York State of Health insurance marketplace. Hospital affiliations: Sisters Hospital and the John R. Offers comprehensive services to children and families, which promotes literacy skills and social, mental, emotional, and physical needs. Parents are the cornerstone of the Head Start Program. The Head Start staff recognizes that parents are the child's first teachers. We encourage parent involvement in all aspects of programming from classroom volunteers to serving on decision-making committees. Provides safe settings, Kindergarten readiness skills, classroom and home-based experience, hot nutritious meals, health screening, community referrals, transportation, non-violent problem solving skills and day care referrals for children and siblings.

A center specializing in Alzheimer's and related dementia day care. Provides social model adult day care, socialization, hot lunch, breakfast and afternoon snack. Services include small group crafts, exercises, field trips, personal care in activities of daily life, ongoing care plans, information and referral, transportation within a 15 mile range, and wheelchair transportation. Inter-generational program with on-site child care offered. Alzheimer's support group held the 3rd Thursday of each month, 7pm-9pm.

Provides a full complement of services to adults ages 55 and over. Daytime programming includes breakfast, lunch, take-home food, exercise, field trips, transportation services, health and wellness information, assistance with senior benefits/issues, recreation and cultural activities, technology classes, workshops, and referral services. Hearts and Hands is a volunteer based non-profit organization that provides free escorted transportation, companionship, and in-home supports for older adults in communities throughout Erie and Eastern Niagara Counties. Our care recipients are often individuals who stopped driving due to age-related medical conditions or for financial reasons. Recipients may also need assistance at home with light yard work and home maintenance, or companionship to renew and refresh their caregiver. Our elderly neighbors are often lonely, isolated, and in need of friendly human interaction. In response to a recent community assessment and a national assessment from AARP, Hearts and Hands has designed our program to address social isolation through all three avenues of service: transportation, household supports, and companionship. Hearts and Hands volunteers may be the only people our care receivers see during a week, and their company is, in and of itself, a bright spot in daily life. Hearts and Hands is a best-efforts program. Services are provided by volunteers who give their time as they have it available. Operates a coach-type bus which serves most of Delaware County. Different fixed routes, operating three (3) days a week, have been established to help riders meet their shopping and social needs. Also offers limited transportation service to meet non-emergency health care appointments and pharmaceutical needs.Transportation arrangements should be made 5 business days in advance. 24 hour notice is requested for cancellations. Four requests per program year are allowed per client.

The CAPteen Program promotes youth development through a variety of leadership, volunteer community service, and character building opportunities. This student led initiative of CAPTAIN CHS offers teens in the Shenendehowa School District a variety of activities that give back to the community.

Serves consumers whose transportation needs cannot be met by Medicaid transportation and to ensure they have access to medical appointments and HIV related services. Individuals are provided cab rides, bus tokens, or gas cards to meet with their medical providers.
