Tuscarora Community Health Worker Program provides home-based case management services to participating women and their children and helps them overcome barriers to care. Workers help identify complicating factors that make it difficult for clients to access the care they need; assist in enrolling pregnant women into prenatal care and children into primary care; assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus; provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence, and substance abuse; provide referrals to food pantries, housing assistance, educational programs, childcare, and other services; and provide support and help to overcome the barriers that prevent families from receiving a healthy start in life.
