Offers a set of home and community based Medicaid State Plan services: Other Licensed Practitioner (OPL), Community Psychiatric Support and Treatment (CPST) and Psychosocial Rehabilitation (PSR), aimed at supporting the social, emotional or behavioral health of Suffolk County youth, ages 5 through 21 and their families/caregivers. Other Licensed Practitioner (OPL) provides therapeutic services to youth by a licensed mental health professional. Community Psychiatric Support and Treatment (CPST) offers client and family support in improving daily living, personal and community relationships. Psychological Rehabilitation (PSR) provides "hands on" assistance and practice of skills acquired through therapeutic or CPST services. Services can be individually accessed or used jointly.

Offers a non-traditional high school program that offers students in grades 9-12 the opportunity to fulfill New York State graduation requirements by earning both academic and Career and Technical Education (CTE) credits. The program is made possible by the collaboration among Baldwin UFSD, Nassau BOCES, and Simon Youth Foundation. The RESTORE Program is an emergency home repair program for homeowners 60 years of age and older and assists residents of Essex County with single-family home repairs through grants up to $10,000. Pre-applications are received at any time and depending on available funds qualified applicants will be chosen on a first come, first served basis.

Offers therapeutic community based residential treatment for youth. Services include medical/dental care, individual, group and family therapy, substance abuse risk assessment and counseling. Each youth has an individualized care plan and addresses academic needs, mental health needs, substance abuse needs and skills necessary to work toward self sufficiency. Each of the homes are supervised 24 hours per day and the staff includes MSW's, Residential Counselors and Bachelor's level professionals. There is currently one adolescent boys home and one adolescent girls home.
