Putnam NY Connects promotes personal responsibilities to empower and create opportunities for consumers and their caregivers of all ages, allowing them to make informed choices about their Long Term Care needs that balance cost, access, and quality. Our aim is to help alleviate the frustrations of trying to navigate through a complex and fragmented long term care system.

Organizes visits to hospitals by volunteer musicians. The musicians perform for patients who are too sick to leave the hospital. Creates CD pharmacies for healthcare facilities as well as help patients write and record original music. Serves all locations.

Offers an opportunity for adults to volunteer as mentors for local youth, from elementary through age 21, to serve as role models, and can commit to least one hour per week.

Provides free legal services and referrals to Latina women in abusive relationships. Also offers a workshop, the first for Latina women, Talleres de Liderazgo, that is a free five-week course, taught in Spanish only, on leadership and basic legal rights training. Offers a comprehensive personalized Travel Medicine Service to meet the needs of travelers going abroad for both business and pleasure. The service provides expert consultations and vaccinations for safe and enjoyable international travel. The Travel Medicine Service serves both individuals and travel groups. A thorough medical history assessment, combined with an itinerary analysis is done to determine the recommended and required vaccinations along with other preventive measures based on destination and length of stay. The Infectious Disease experts in the Travel Medicine Service at NUMC utilize the most up-to-date travel disease databases to administer required and recommended vaccinations, provide an International Certificate of Vaccination, and provide necessary prescriptions for preventive medications. The complete range of vaccinations available to meet international travel requirements or recommendations includes yellow fever, hepatitis, typhoid, polio, meningitis, rabies, varicella, and Japanese encephalitis, as well as prescriptions for malaria prevention and treatment of travelers’ diarrhea. An acute and general psychiatric inpatient unit for adults dealing with various mental or emotional disorders.

Provides a warmline which offers peer support, information, and referrals for those with mental health needs, questions or concerns. Also helps with meeting individual goals (education, fitness, employment, etc.). The warmline is manned by recipients of mental health services. Note: Voicemail will direct caller to specific extension for peer support staff based on day of the week.

Preparing Youth for Adulthood (PYA) Program is designed to prepare our young people, ages 14 to 20, for the transition from foster care to self-sufficiency.

Accept donations of clean, used, in-season clothing and small items for re-sale at low prices to benefit the mission program of our church. Please enter thrift shop through the stairwell located to the right of the front doors of the church. Donations are accepted ONLY during hours of operation. DO NOT leave in stairwell or hallway of thrift shop, as they will be discarded.
