Certified by the New York State Office of Alcoholism and Substance Abuse Services (NYS OASAS). All services are strictly confidential. The Niagara Falls facility is located on the corner of Third Street and Rainbow Avenue. Park in the side lot and enter through the door facing 3rd Street. An effort is made to provide all available care options so as to enable consumers to remain at home. Also conducts screenings to determine eligibility for different services. Will conduct in-home assessments as necessary. Serves as the point of contact for the Senior Farmers' Market Nutrition Program, which provides low-income seniors with coupons that can be exchanged for eligible foods (fruits, vegetables, honey, and fresh-cut herbs) at farmers' markets, roadside stands, and community supported agriculture (CSA) programs.

A comprehensive managed long term care program for older adults who wish to live at home but need assistance in order to maintain health, complete day to day activities, and remain safe at home for as long as possible. Primary and specialty medical care, adult day center, meals, prescriptions, hospitalization, and emergency services provided. In-home support, physical and occupational therapy, and transportation are also available.

Provides community meeting space, food pantry referrals, special events, family activities, and technology center / computer lab. Senior programming includes exercise, movies, parties, senior council meetings, bingo, nutrition program for seniors, and field trips. Youth programming includes an after school program for children 5-10 years old, 4-H programs, homework help, teen and adolescent life skills, youth and teen recreation, summer day camp, and homework assistance. Also provided are youth sports, adult sports and recreation, adult athletic leagues, exercise programs, and a fitness room. Serves as a City of Buffalo Cooling Center during times of extreme heat and as a Warming Center during times of extreme cold. Always call ahead to make sure a Cooling/Warming Center is open before going there.

A program designed to help parents meet the challenges of parenting young children and to ensure their children's healthy development. Program provides comprehensive, home-based services to families during pregnancy or with newborns up to three months of age. The family can continue in the program until the child is five years old or enters Head Start. Services of a Certified Lactation Consultant, Doula, and/or Father Advocate can be included as a complimentary add-on for families. A support group for fathers is also offered.
