Coordinates care for individuals dealing with substance use, including providing connections to mental health, chemical dependency, and rehabilitation services, as well as primary care, counseling, support groups, and employment and food assistance.

Provides regularly scheduled intensive in-home visitation by paraprofessionals who provide advocacy, socialization, and referral services for adult victims of elder abuse and domestic violence.

The Office for Aging offers outreach to seniors who are unable to understand or access the information or services that they need. They visit homes, write newspaper articles, speak to groups, send information to doctors' offices and churches and refer seniors to as many programs as they need.

* Reduces the prevalence of high lead levels in children.* Provides community education and tracking.* Offers lead testing to clients who don't have a physician or medical insurance.* Offers case management for children identified with lead poisoning.

* Provides home health aides, licensed nurse practitioners, registered nurses, attendants, and personal care aides to work and oversee medical treatment in the home. Some of the services provided are: Medication reminders Grooming and oral hygiene Bathing and dressing Mobility issues Meal preparation Laundry Housekeeping Errands Companionship* Provides a full range of services under the TBI/NHTD Waiver that includes the following: Home and Community Support Staff, Community Transitional Services, and Moving Assistance and Transportation. * Offers non-medical transportation services.* Provides care management through Health Homes of Upstate NY (HHUNY) and Children's Health Homes of Upstate NY (CHHUNY).* Offers tuition free certification for Personal Care and Home Health Aides.

Children from birth through age 21 enrolled in Medicaid and have one or more of the following:1) At least two chronic conditions2) Diagnosed Severe Mental Illness (SMI) including Severe Emotional Disturbance (SED) or3) HIVPending approval from CMS, "trauma, and at risk for another chronic condition" will be added to these eligibility criteria.

The YES program is designed to address truancy and at-risk behaviors by working one-on-one with youth through positive communication and mentorship, and promoting healthy futures for Chenango County youth and their families.

* Offers information and referral regarding issues of aging such as: Long term care services and supports information. Aide service, home delivered meals, transportation, medical alarm units, nursing homes. Caregiver support, housing, employment supports, etc.

Assists in identifying appropriate services, linkage and referral to recovery-oriented services and other services in accordance with goals identified by the recipient in a written case management plan.

Offers specialized planning for families for all aspects of school, medical, legal and financial for all ages and stages of life.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

Provides programming focused on academic enrichment, such as homework help and tutoring, school engagement, dropout prevention, career exploration, and intensive intervention and case management.

* Offers short term, intensive services to avert unnecessary psychiatric hospitalizations of children and youth through clinical interventions with the child/youth and their families, and case management services such as referrals to longer term services.

* Works with high-need women and families to access preventive and primary health care services, with an emphasis on pregnant women who are not enrolled in health care or supportive services.

Provides survival aid to youth through delivery of gateway services that may include food, beverages, clothing, hygiene and other essential needs. Offers education to youth with brochures, palm cards, and safe sex kits. Provides assessment, case management, prevention activities, referrals, crisis stabilization and follow-up support to youth who are homeless, at-risk of homelessness or who are being trafficked or at-risk of trafficking.

* Offers youth in Rome and Utica opportunities to build their self-confidence, reduce negative social behaviors, increase connectedness to self, home and community, increase employability, and provide community services through an adult mentor.

* Offers learning and enriching experiences through Chandler Day Habilitation Center through a variety of excursions and activities.* Oversees seven supervised residential settings.* Provides Community Habilitation Services to individuals with various services designed to assist individuals in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to interact in the community.* Provides employment services to individuals through SEMP, ETP, and Pre-Vocational Services.

* Coordinates services related to medical, mental health, substance use, and social needs. Works closely with community providers, e.g., schools and mentor programs, to promote success and independence in children and family stability.

Assists individuals who have HIV/AIDS or other chronic illnesses in receiving housing, medical services and medication needs, receive social services regarding food, arranging transportation.

Provides peer support to individuals seeking or maintaining mental wellness by identifying strengths and passions for continued wellness, and community participation. Peer support is available one-to-one or in group settings.

Assists individuals with coordination needs such as goal setting, health promotion, transitional care, referrals to community and social supports and patient and family supports, and advocacy. Individuals work with a primary care manager to create their own plan of care.
