This program utilizes a wrap-around model of service delivery designed specifically for children and adults to assist with linking individuals and families to formal and informal supports and services in the community. Case management can help in the area of living arrangements, physical health needs, mental health needs, economic needs, vocational/educational needs, social functioning, family functioning, self-care needs, self-direction, and substance abuse.

Staff is located on site who advocate for residents; connect them to services available in the community; provide independent living skills training, and facilitate social and recreational opportunities.
