Offers a community based program that provides mental health services in the home or community. Services include counseling, medication management, peer services and case management. Community Residences are home-like programs for youth with mental health needs. CRs offer supervision, structure, care coordination, skill building and behavior support. The goal of a CR is to assist youth in achieving their best level of functioning so that they can be successful in school and at home when they return to their home and community. Residential Treatment Facilities are therapeutic programs that provide inpatient psychiatric treatment to youth with complex mental health needs and support to their families. The youth have challenges that need around-the-clock supervision, structure, and clinical treatment. Please call for detailed information regarding both programs offered

Provides an after school program that includes activities such as drama, art, dance, robotics, math, and other fun academic activities to enrich and encourage creativity, sharing of ideas, drawing conclusions, discussion and problem solving. LEAP is committed to creating activities and making community connections.

Offers a variety of support groups for families and friends of those with mental illness.

Provides emergency medical care and transportation to a medical facility, for those in the local community, due to illness, accident, or other medical emergency situation.

Peers are individuals with lived experience. Peers are available in the community and integrated into existing services (ex: hospitals, outpatient clinics, etc.) to support individuals on their positive pathways to behavioral health and wellness. Our peers serve as educators, advocates, supporters, and connectors to other resources.

Provides free or low cost subsidized child care services for eligible families residing in Suffolk County. Child care is available for children, between the ages of 6 weeks through 12 years in licensed day care centers, school age child care programs, group family day care homes, registered family day care homes, and legally exempt/informal child care programs. NOTE: Children with special needs may be able to receive subsidized child care through 18 years of age. Families must meet income and program eligibility criteria to be eligible for a child care subsidy. Provides Information and Referral, Reference Services, Readers Advisory and programs for children from birth through grade 8 as well as parents. Programs and services include, but are not limited to, topics such as parenting workshops, storytimes, reading clubs and arts and crafts.

Offered year-round, the program matches adult mentors with youth ages 18-24 from the community to serve as role models and to offer personal and academic guidance, support and encouragement. Includes one-on-one, group and peer mentoring.

Provides low-income housing for individual, families, and seniors who otherwise cannot afford housing.

Skilled nursing facility of 252 beds providing alzheimer's disease/dementia unit, hospice care, intensive rehabilitation services including physical, occupational and speech therapy, long term care, short term rehabilitation, respite care, wound care, and I.V. Therapy.

Provides various general medical services such as routine physical exams, well baby care, immunizations, etc.

Provides medical and sexual health services to gay, lesbian, bisexual and transgender individuals, including referrals for other resources and services in the community.

Provides Medicaid service coordination for individuals with a developmental disability who meet the eligibility criterion of NYS Office of People with Developmental Disabilities(OPWDD). Also will assist those eligible to receive Medicaid under the OPWDD system.

Offices provide convenient one-stop shopping for employment-related needs. Division of Employment and Workforce Solutions offices offer many DOL programs in a single location - including career related assistance and services for employers. Staff at these centers have been cross-trained in Department services so they can address more completely the needs of employers and job seekers. The Division of Employment Services works with the public and private sectors to create job opportunities, offer job fairs, help workers find a job, and help businesses find skilled workers. Offers outpatient evaluation and treatment services to individuals with communication, voice, or cognitive problems, including a number of different therapies for swallowing disorders.

Offers information on a variety of crime prevention issues, programs or services, from public awareness, protection, and possible ways to prevent crime, etc. Also provides a helpline for those who have been crime victims.
