Provides comprehensive treatment, recovery, and support services for individuals and families impacted by mental health and substance use challenges. Services offered include individual and group counseling, medical and psychiatric care, family support coordination, addiction medicine consultations, opiate overdose prevention training, recovery coaching and peer support services, care coordination, community outreach and education, harm reduction services, a therapy dog program, and more. Description: Federally Qualified Health Center (FQHC) offering pediatrics, internal medicine/family medicine, OB/GYN, pediatric and adult dental services, podiatry, social work, nutritional counseling, medications for opioid use disorder, vision care, and pharmacy with home delivery. Serves all populations with special focus on the medically under-insured, uninsured, and those experiencing homelessness. Also provides onsite lab, ultrasound, STD testing, COVID-19 testing and vaccines, health education/groups, blood pressure screening, and free walk-in pregnancy testing. Provides transportation assistance to help clients get to appointments and referral assistance. Participates in 340B drug discount program and has onsite enrollers for the New York State of Health insurance marketplace. Hospital affiliations: Sisters Hospital and the John R. Oishei Children's Hospital.

24-unit apartment complex with one bedroom apartments; two apartments are fully accessible. Rental subsidies available.

Cash assistance provided to families on a temporary basis while they are in transition to social security disability.

1, 2, and 3 bedroom apartments. 5 units fully accessible for mobility impairments and 3 units accessible with hearing and/or vision impairments.

Provides assistance in the following three categories: 1) Referral: Provides information and referral to services. 2) Representation: Acts as a link between disabled persons, the community, and government agencies. 3) ADA Oversight: Deals with ADA compliance and issues, and works with disabled Erie County employees regarding returning to work with reasonable accommodations. Home Based and Classroom (center based) services are provided for pregnant women and children between birth and five years of age throughout Chautauqua County. Head Start services children ages 3-5.

Offers 2-year degree and 1-year certificate programs in health sciences, business and public service, engineering and technologies, liberal arts, sciences, micro-credentials, and certificate programs including university parallel programs and transfer articulation programs that are NYS Education approved.
