This program consists of a combination of programs and services designed to improve education and access for community members working toward optimal health and well-being. In partnership with health care providers, Community Health Workers provide intake, referral, navigation and case management services to patients at Primary Care Practices.

We operate two community residences in Nassau County and our Massapequa site. The services and supports provide residents with skills to move on to more independent housing programs.

YAP continually evolves services to meet new needs that emerge for youth involved within the juvenile justice system, but most of our programming is designed for the purposes below. Diversion: to divert youth from formal system involvement. Pre-Adjudication: to prevent formal detention while ensuring community safety. Post-Adjudication: to prevent incarceration in prisons, jails or other residential or secure facilities. Re-Entry: to promote safety and support to youth transitioning back to the community from institutional placement.

Offers programming as a response to injuries and deaths caused by drunk drivers. Provides intervention services focusing on enforcement, adjudication, rehabilitation and education. The program is entirely funded through drunk driving fines received through local town, village, city and county courts.

Operates Kindergarten through 8th grade classes.

Responsible for developing, maintaining and operating the system of aqueducts, reservoirs, channels, pipelines, mains, laterals, curb boxes and pumping and purifying equipment by which the community's water supply is obtained, treated, stored, and distributed to consumers.

Evaluating and treating the brain-behavior relationship.

Offers a summer day camp for children ages 5-13 with a number of fun, recreational and education activities and events. Includes free breakfast and lunch..

Assesses and maintains records of all properties in the City of Syracuse.

Provides obstetric and gynecological services for women of all ages and information, diagnosis, treatment, and counseling for the control of sexually transmitted diseases. Offers confidential HIV counseling, and testing.

Provides Reference, Information and Referral, and Readers Advisory. Range of programs and services include, but are not limited to, fitness, arts and crafts, babysitting workshops, Live Librarian and SAT Preparation Workshops.

PREP is a teen pregnancy prevention program for middle school-aged youth that uses evidence-based program models to educate youth on healthy decision-making patterns, abstinence and contraception, as well as adult preparatory subjects, which includes healthy relationships, financial literacy, effective parent-child communication, and college and career planning. PREP has an Advisory Board that is dedicated to helping through cross-collaborative partnerships (related to positive youth development work), program oversight, sustainability, and expansion opportunities for youth in Haverstraw and Spring Valley.

Provides outpatient mental health services, such as mental health screening, individual, group, child/adolescent counseling, geriatric services, including after care support.

Year-round programming for middle and high school youth, primarily Newfield residents. Provides skills, education and other services to help individuals with psychiatric disorders to transition from hospitalization, or long term illness back into the activities of every day living.

Tuesday 9 AM- 12 PM for Veterans, Wednesday 2-7 PM for everyone
