Provides door-to-door transportation for seniors five days a week. Lunch is provided M-F Noon-1pm. Call for information. This program is funded by the Nassau County Department of Senior Affairs as a Multi Service Senior Center.

Outpatient medical facilities operated by the United States Veteran's Administration that provide routine primary medical care for eligible veterans. Services generally include x-rays, general health check ups, blood pressure checks, treatment for basic illnesses and laboratory tests.

A visitor can use any of the materials in the library within the library, including our databases. You may be eligible to borrow through one of our Courtesy Borrower programs. Courtesy borrower’s privileges may be extended to patrons who are not enrolled or employed at Stony Brook University. Our Community Borrowing program extends borrowing privileges to Stony Brook University alumni, local (Nassau and Suffolk County) health practitioners and individuals with Community Borrower Cards for an annual fee. Participants in our Courtesy Borrowing program are eligible to borrow library books at all campus libraries. Other resources such as interlibrary loan, off-campus access to databases and recalls may not available to guest borrowers. Individuals wishing to have borrowing privileges at Stony Brook University Libraries may have guest borrowing privileges by completing a Community Borrower Program Card Application. Offers summer day camping for children 2-17 years old preschool camp held at Mid Island Y, day camp for grades K-6 held at Henry Kaufmann Campgrounds in Wheatley Heights, special teen and travel programs for 7-11th grade. Before and after care provided if needed.

Offers transportation for senior citizens through the Dial-A-Ride program within the Town limits for weekly food shopping, monthly banking and medical appointments. Offers Home Aide program which provides light housekeeping, shopping, laundry and errands for eligible seniors. Minor home repairs are also available. Homeowners only pay for the materials, not labor. Offers Meals on Wheels program which delivers hot noontime meals on weekdays to homebound seniors and/or those unable to cook for themselves. Sexual Health and HIV Clinics provides a clinic for anonymous and confidential HIV testing and counseling, and sexually transmitted disease testing and treatment.

Provides assistance to Medicaid eligible individuals with developmental disabilities and their families to access resources and services within the community, along with coordinating the necessary services, and to navigate the system to get benefits and other entitlements.

Open Door provides Outreach and Enrollment staff that are committed to enrolling people in insurance programs that fit their budgets. Coverage is often available regardless of citizenship or immigration status.

A maternal-child nurse is available for free home visits to the families of newborns. Nurses provide instruction about parenting skills, breast or bottle feeding, baby baths, and other issues related to the care of infants, and assessment of health of newborn and post-partum mother. Maternal-child nurse also available for home visits to pregnant women to teach and monitor prenatal care. Nurses available for home visits to children who are ill.

MHAW: Mental Health Helpline is available to those who need assistance to access behavioral health services and information for themselves, and family or friend, or community group

Wardrobes for Work, has been providing proper work attire to individuals seeking or maintaining employment. The joint venture formed by business and human service organizations in Sullivan County, operates under the guidance of United Way of Sullivan County and in conjunction with county work programs.

Prevention specialists work in the schools and community using evidence-based practices that focus on substance abuse and other destructive behaviors.

Provides care for individuals who have no income or insurance, and other qualifying information.

Provides access to individuals and families who are new to this country through a network of education, advocacy and referral services to language skills, business planning, loan information, community engagement, and more.

Collects and distributes new and used donated clothing to walk-in individuals.

Program connects pro se clients with trained attorneys to assist them in filling out the forms and any ancillary legal documents appropriately. LSHV and/or a volunteer attorney could follow up with the clients assisted in the clinic to determine whether the divorce papers were filed correctly.

Allows handicapped residents to borrow items such as walkers, cane, wheelchair, and potty wheelchair at no cost. Provides applications to purchase expensive equipment.

A free public elementary school for students in grades Kindergarten through 5th living within district boundaries.

Provides financial assistance for low income families with children under 18.

Offers outpatient clinics in various areas around the county to provide primary medical care and urgent care for children and adults.

Provides classroom based programs for children ages 2.9 through 5 years of age, who have been evaluated and approved to attend the program by the Committee on Preschool Special Education (CPSE) from the community in which the child resides. As per the child's Individualized Education Plan (IEP) children may receive rehabilitative therapies, nursing services and counseling services. Also offers a day care component for typically developing children. The Center is also an approved evaluation site for the Committee on Preschool Special Education, with evaluations being conducted on site in either Bayside or the Roslyn site (St. Mary's Kids at Roslyn) as well as in the child's home. Provides resources for optimal housing, health and education and serves as a catalyst for sustainable change through the development and implementation of dynamic programs as well as strategic partnerships with leading nonprofits and community organizations.

Provides a six-week respite program, during the summer for children with and without disabilities a chance to take part in a variety of activities. Campers are grouped by age to fully enjoy age-appropriate activities. Activities include swimming, sports, arts and crafts, theater, music, dance, nature walks and outdoor adventures and much more.
