Adult Home: Provides residential care, room and board, housekeeping, activities, laundry, personal care, and supervision in a home-like environment. All residents must be private pay. Assisted Living Program: Provides the services above with additional personal care and supervision provided by nursing and home health aides. Assists individuals to remain independent in their homes and educates families on various health issues providing a greater understanding which leads to informed decision making. Provides a weekly respite care program for adults at Christ Episcopal Church in Albion, NY. Provides respite for caregivers with organized activities, support, and supervision for those they care for.

Provides individually customized support by licensed grief professionals, acknowledging that grief affects everyone differently. Anyone is eligible to call and receive support. Supportive services include telephone support, counseling referrals, group referrals, youth support resources, resource library, and educational presentations. As an affiliate of Hospice & Palliative Care Buffalo, also provides counseling and support groups for those in the hospice bereavement program.

Provides one-on-one assistance for individuals interested in shopping for and enrolling in health insurance through The New York State of Health - the Official Health Plan Marketplace. Navigators are now available to speak with individuals and families to provide education about the different types of health insurance plans offered through the New York State of Health (NYSoH) Marketplace and to assist with enrollment into public insurance programs and qualified health plans.

Provides organized coordination and management of medical, mental health, substance abuse, and social service needs to Medicaid individuals with chronic conditions. Provides guidance to help families and their children make informed decisions about their care and improve health, well-being, and quality of life. Available to children and youth under 21 who are covered by Medicaid and have mental health and/or substance use needs. Therapeutic and Rehabilitation Services are based on a full assessment, and are provided at home or in the community. Services will be provided by a Credentialed Family Peer Advocate/Certified Recovery Peer Advocate who has had similar experiences.

Hearts and Hands is a volunteer based non-profit organization that provides free escorted transportation, companionship, and in-home supports for older adults in communities throughout Erie and Eastern Niagara Counties. Our care recipients are often individuals who stopped driving due to age-related medical conditions or for financial reasons. Recipients may also need assistance at home with light yard work and home maintenance, or companionship to renew and refresh their caregiver. Our elderly neighbors are often lonely, isolated, and in need of friendly human interaction. In response to a recent community assessment and a national assessment from AARP, Hearts and Hands has designed our program to address social isolation through all three avenues of service: transportation, household supports, and companionship. Hearts and Hands volunteers may be the only people our care receivers see during a week, and their company is, in and of itself, a bright spot in daily life. Hearts and Hands is a best-efforts program. Services are provided by volunteers who give their time as they have it available. Assisted living facility that provides 24 hour supervision, case management, meals, personal care, housekeeping, laundry, activities, transportation service, and assistance with medication requirements. STEL also has an adult home in Fredonia under the auspices of the Department of Health.

Recreation programs include gym and game room activities, pool and ping-pong tables, games, arts and crafts. Also hosts the South Buffalo Community Association senior center (see separate record). Serves as a City of Buffalo Cooling Center during times of extreme heat and as a Warming Center during times of extreme cold. Always call ahead to make sure a Cooling/Warming Center is open before going there.

Day program for older adults and adults with developmental disabilities. Provides socialization, community involvement, nutritional program, transportation, recreation, and case management. Helps older adults and adults with developmental disabilities remain at home and gives caregivers time away from caregiving to help them do whatever they need to do, whether that be running errands, attending appointments, or going to work. Respite services are also offered at this location; services can be provided for as little as one hour to a full day.

Provides dental care, pediatric care, and primary care (family medicine) in a certified Patient Centered Medical Home that is accessible and focused on quality and safety. Can help enroll in health insurance coverage.
